回肠代膀胱术后患者代膀胱肠黏液分泌规律的研究

摘　　要：目的　探讨膀胱全切、回肠代膀胱术后代膀胱内肠黏液的分泌规律和特点 ,以指导临床护理工作 ,防止术后并发症。方法　从术后第 3天至第 16天 ,对 2 0例行膀胱全切、回肠代膀胱术后患者代膀胱内的肠黏液进行测量、计算和统计。结果　代膀胱内肠黏液的分泌量在术后第 3天最少 (平均 10 6 .18mg/cm2 ) ,术后第 7天最多 (平均 2 30mg/cm2 ) ,以后逐渐减少。结论　代膀胱内肠黏液的分泌量具有规律性 ,呈现出上升—高峰—下降的趋势 ,但并不消失 ;护理工作中应相应增减膀胱冲洗的次数 ,即术后第 3天每日冲洗 1～ 2次 ,至术后第 7天左右增至每日 3～ 4次甚至更多 ,以防止引流管堵塞、尿液排出不畅等并发症的发生。Objective To explore the regularity of intestinal mucus secretory after orthotopic reconstruction of bladder with ileum operation.Methods The weights of intestinal mucus in 20 patients after orthotopic reconstruction of bladder with ileum operation were measured.Results On the third day of post-operation,the weight of mucus was the lowest;on the seventh day of post-operation, the weight of mucus was the highest.Conclusions The secretory of intestinal mucus has its regularity. Nurses should improve the frequency of bladder irrigation ,especially on the seventh day post-operation.
