老年糖尿病患者颈动脉病变与血脂关系的研究

摘　　要：目的　探讨老年糖尿病患者血脂水平与颈动脉血管并发症的关系。　方法　老年糖尿病患者 15 0例为糖尿病组 ,年龄 6 0～ 89岁 ;10 6例同期健康体检者为对照组 ,年龄 6 0～ 88岁。受试者均测定空腹血压、血糖、胆固醇 (TC)、甘油三酯 (TG)、高密度脂蛋白胆固醇 (HDL C)、低密度脂蛋白胆固醇 (LDL C)、极低密度脂蛋白 (VLDL)、载脂蛋白AI(apoAI)和载脂蛋白B(apoB)同时行彩色多普勒颈动脉超声检查。　结果　 (1)糖尿病组血糖、TC、TG、VLDL、apoB〔(9 81± 3 2 5 )mmol/L、(5 2 7± 1 10 )mmol/L、(5 32± 3 78)mmol/L、(0 86± 0 84 )mmol/L、(2 5 8± 0 89)mmol/L〕水平高于对照组〔(6 96± 0 0 9)mmol/L、(4 5 8± 0 94 )mmol/L、(3 0 3± 1 5 7)mmol/L、(0 5 7± 0 30 )mmol/L、(2 0 9± 0 5 9)mmol/L〕 ,两组比较差异有显著性 (P <0 0 5 ) ,糖尿病组纤维蛋白原为 (4 6 7± 1 6 4 ) g/L ,高于对照组 (3 2 4± 1 0 7) g/L ,P <0 0 1;(2 )老年糖尿病患者的颈动脉超声显示颈动脉血管壁回声不均匀 ,内中膜增厚 ,可有多支血管斑块形成及不同程度的狭窄。　结论　脂代谢异常是血管并发症形成的基础 ,颈动脉粥样硬化斑块是糖尿病患者早期主要血管病变之一 ,颈动脉超声能提示老年糖尿病患者的?Objective To evaluate the correlation of hyperlipidemia and carotid arthrosclerosis manifestations on color Doppler ultrasonography and dynamics of elderly diabetes patients. Methods One hundred and fifty elderly diabetes patients and 106 normal subjects were enrolled in the study. The carotid color Doppler ultrasonography and blood sugar, TC, TG, HDL-C, apoB,VLDL, apoA I, apoB were examined and compared between the two group. Results The blood sugar, TC, TG, HDL-C, apoB, VLDL, apoA I, apoB were increased in diabetes patients which were higher than in normal subjects (P<0.05). The thickness of intima-media (IMT) of carotid were increased significantly, artery narrowing and wall plaque were also found more than normal subjects. Conclusions The arteriosclerosis plaque is one of the early vescular lesions in the elderly diabetes patients and might be caused by hyperlipidemia. The carotid dopper ultrasonography shows the early arteriosclerotic changes and offers the basic data for clinical treatment.
