中药加味参芪仙补汤为主治疗小儿慢性再生障碍性贫血的研究

摘　　要：目的　探讨小儿慢性再生障碍性贫血 (简称再障 )临床特点及中医中药治疗方法。方法　对 1993— 2 0 0 2年我院收治的 16 2例小儿慢性再障的临床特征进行回顾性临床分析 ,对加味参芪仙补汤对其辨证治疗的疗效进行观察。结果　小儿再障多发于学龄前及学龄期儿童 ;男性多于女性 ;临床以贫血、出血症状为主 ;外周血以血小板减少最为显著 ,白细胞计数约 5 0 % >4 0× 10 9L-1,网织红细胞百分比计数约 5 0 %在正常范围 ,但绝对值绝大多数明显减少 ;骨髓增生活跃病例多于增生低下者 ;T淋巴细胞亚群绝大多数异常 ,以CD3 ,CD4阳性细胞减少 ,CD8阳性细胞增高 ,CD4/CD8比值减低为特点 ;免疫球蛋白绝大多数异常。加味参芪仙补汤为主治疗小儿慢性再障总有效率为 87% ,治愈缓解率为 6 0 % ,疗效显著。结论　小儿慢性再障有其自身的规律与特征 ;中药为主中西医结合治疗小儿再障疗效好于单纯西药治疗。Objective It is to discuss the clinical characteristics of children with chronic aplastic anemia and its treatment of traditional Chinese medicine with herbs.Methods 162 children with chronic aplastic anemia who had been treated in our hospital during 1993 to 2002 were taken retrospective clinical analysis, and the curative effect of Jiaweishenqixianbu decoction was observed.Results Children got affected by aplastic anemia usually at before or during school ages. Male patients were more than female cases. Anemia and hemorrhage were the main symptoms. The reduction of platelet was the most remarkable change in peripheral blood. The leukocyte counts of about half of them were more than 4.0×109 L -1. The reticulocyte percentage counts of about half of them were in normal range but the absolute value of the majority clearly declined. The cases with active hyperplasia of marrow were more than those with fallow hyperplasia of marrow. The T lymphocyte subsets of most of them were abnormal. The characteristic included the reduction of CD 3, CD 4 masculine cells, the increase of CD 8 masculine cells and the decrease of the CD 4/CD 8 ratio. The immunoglobulins of most of them were also abnormal. The total effective rate in this group was 87%, and the cure and remissive rate was 60%. The curative effect was remarkable.Conclusion The chronic aplastic anemia of children has its own rule and characteristic. The curative effect of integrated traditional Chinese and western medicine with Chinese medicine in major is better than that of western medicine only.
