普鲁泊福复合芬太尼用于人工流产术的疗效观察

摘　　要：目的　观察普鲁泊福复合不同剂量的芬太尼用于人工流产术的效果。方法　 84例人工流产患者随机分成四组 ,每组 2 1例 :P组以普鲁泊福 3 .5mg/(kg·h)速率输注 ,F1 ,F2 ,F3组则于输注普鲁泊福前 1min分别静注芬太尼 0 .2 5 μg/kg ,0 .75 μg/kg ,1.2 5 μg/kg。各组病人分别监测手术前 (S1 )、扩宫时 (S2 )、扩宫结束 (S3)、吸宫时肢动出现 (S4 )时的平均动脉压 (MAP)、心电图 (ECG)、心率 (HR)、呼吸 (RR)、脉搏、氧饱和度 (SpO2 )和普鲁泊福的首剂量、总用量、呼吸抑制发生率、术毕意识恢复时间和定向力恢复时间。结果　F3组比F2 组呼吸抑制发生率明显增多 (P <0 .0 5 ) ,F3组意识恢复时间较其他组明显延长 (P <0 .0 1) ,P组和F1 组普鲁泊福首剂量和总用量明显增多 (P <0 .0 1)。结论　普鲁泊福复合 0 .75 μg/kg的芬太尼既可加强镇痛、减少普鲁泊福用量 。Objective To study the effect of propofol anesthesia plus different dosage of fentanyl in induced abortion. Methods Eighty-four patients, who would accepted induced abortion, were at random divided into four groups. Group P (n=21) was administered propofol (3.5mg/kg·h) only. In addition to propofol, group F 1, F 2 and F3 were respectively given with 0.25, 0.75 and 1.25 μg/kg of fentanyl. Inductive time, recovery time, and breath repression were recorded. Results Propofol dosages of group F 2 and F 3 were less than that of group P (P<0.05). The rate of respiratory depression of group F 2 was lowest.Conclusion Propofol plus fentanyl for induced abortion anesthesia may enhance analgesia and reduce the propofol dosage. Propofol plus 0.75 μg/kg's fentanyl is a safe and effective protocol for induced abortion anesthesia.
