利多卡因明胶纤维网在烧伤取皮部位止血、止痛的临床观察

摘　　要：目的 :观察 2 0 %利多卡因明胶纤维网用于烧伤取皮部位止血、止痛的效果。方法 :将烧伤植皮病人 2 6例随机分为A、B两组 ,A组用 2 0 %利多卡因明胶纤维网 ,B组用医用凡士林油纱 ,分别在取皮部位加压止血 ,观察止血效果 ;并采用视觉模拟评分法 (VAS)对两组病人术后 4、8、12、16、2 0、2 4、2 8、3 2、3 6小时的镇痛效果进行评价。结果 :A组平均出血量明显少于B组 ,不同时间疼痛评分也显著低于B组 ,经统计学处理有显著性差异 (P <0 0 1)。结论 :2 0 %利多卡因明胶纤维网不但止血效果优于医用凡士林油纱 ,且具有缓释止痛作用。Objective:To observe the analgesic and hemostatic effects of 20%-Lidocaine gelatin fleece on traumatic bleeding after dermatoplasty.Methods:26 burn patients undertaking dermatoplasty were randomly divided into two groups, A and B.In Group A, 20%-Lidocaine gelatin fleece was used while vaseline gauze was applied to Group B. The two materials were put on the wounds after skin grafts were taken.Then pressure hemostasis was performed.The hemostatic effect was observed and Visual Analogue Scale (VAS) was adopted to evaluate the analgesic effect 4,8,12,16,20,24,28,32 and 36 hours after dermatoplasty.Results:In Group A, the average amount of bleeding was less than that in Group B; the scores of pain were lower in Group A than in Group B at different time points. There were statistically significant differences between the two groups (P<0.01).Conclusions:20%- Lidocaine gelatin fleece had not only better hemostatic effect but also better sustainable release analgesic effect over vaseline gauze.
