摘 要:目的 应用气相色谱 质谱 (GC MS)法测定丙泊酚血药浓度 ,比较不同温度、不同保存时间的丙泊酚血浆和全血浓度。方法 研究分两部分 :临床选择 8例下腹部择期手术患者 ,ASAⅠ~Ⅱ级 ,丙泊酚靶浓度设为 3μg/ml实施靶控输注 (TCI) ,TCI开始后 10、2 0、30、4 0min分别采集动脉血 3ml,血标本被分成 3等份 ,分别立即、静置 1h和静置 3h后离心 ,- 70℃下保存 ,2d内测定。体外研究由健康志愿者提供全血 4 8ml,平均加入 4支抗凝试管中 ,并分别保存在 2 4℃和 4℃下。丙泊酚标准品加入试管中分别配成浓度为 4 μg/ml和 2 μg/ml,持续轻摇混匀 ,混匀 3、5、10、30、6 0和180min后分别取 1ml全血和 1ml血浆标本 ,并应用GC MS法测定。结果 临床研究中 ,静置不同时间后离心的丙泊酚血浆浓度的差异无显著性。在体外研究中 ,不同的保存温度 (4℃和 2 4℃ )和保存时间的丙泊酚血浆和全血浓度的差值百分比差异无显著性。结论 保存时间和温度的差异不影响丙泊酚血浆和全血浓度 ;GCObjective To compare the measurement of propofol concentrations in plasma with whole blood in different centrifugal duration, temperatures and storage times by using gas chromatography-mass spectrometry (GC-MS).Methods Experiment was divided into two parts. In clinic experiment, target-controlled infusion of propofol was administered to ASA Ⅰ-Ⅱ scheduled operation of 8 patients. A single constant target plasma propofol concentration was 3μg/ml. Blood samples (3 ml), which were obtained after TCI 10, 20, 30 and 40 min, were placed in three heparin anticoagulant tubes. And they were centrifuged during different placed duration (i.e. instantly, after 1 h and 3 h). Plasma samples were stored at -70℃ until analyzed for propofol contents. In vitro study, 48 ml of blood samples were obtained from healthy volunteers and divided into four heparin anticoagulant tubes. Two tubes were stored at 24℃ and the others at 4℃. Propofol (4 μg/ml and 2 μg/ml) was added to each tube and shaken gently, continuously. Then 2 ml aliquots of blood were taken at 3, 5, 10, 30, 60 and 180 min after mixing, for concentration measurement by GC-MS. Results The differences of propofol concentration in plasma during centrifugal duration after different storage times were not significant. And the differences of propofol concentration in plasma or blood during different temperatures and storage times were not significant.Conclusion The differences of temperatures and storage times had no effect on propofol concentration in plasma and blood. Either the plasma or blood samples by using GC-MS were to determine accurately propofol concentration.
