导乐分娩结合笑气吸入对分娩镇痛及妊娠结局的影响

摘　　要：①目的　研究导乐分娩结合活跃期吸入笑气对分娩镇痛、分娩方式及新生儿的影响 ,以探讨其临床应用价值。②方法　将同期住院可经阴分娩的正常初产妇 4 6 5例随机分两组 ,观察组 (2 10例 )以导乐分娩结合活跃期笑气吸入 ,对照组 (2 5 5例 )未采取任何措施按常规分娩。对两组产妇的疼痛程度、剖宫产率、助产率及总产程、新生儿窒息率等进行比较。③结果　观察组产妇疼痛程度、剖宫产率显著低于对照组 (χ2 =10 8.4 0 0、8.173,P <0 .0 1) ;观察组经阴分娩者的总产程明显较对照组缩短 (u =9.16 9,P <0 .0 1) ,其助产率亦明显低于对照组 (χ2 =3.90 8,P <0 .0 5 ) ;两组新生儿窒息率相比无显著性差异 (χ2 =0 .198,P >0 .0 5 )。④结论　导乐分娩结合笑气吸入可明显减轻产时疼痛 ,缩短产程 ,降低剖宫产率 。Objective To evaluate the effects of Doula delivery with inhalation of laughing gas during accelerating phase in terms of delivery analgesia, the way of delivery, the influence on fetus, and the value of clinical application. Methods Primi- parae of 465 suitable for vaginal delivery were randomly divided into two groups. The 210 primiparae in the observation group received Doula delivery with inhalation of laughing gas during accelerating phase, while the 255 primiparae in the control group were given routine vaginal delivery. The analgesia effects, cesarean section rate, the aid-vaginal birth rate, total delivery process and the anoxia rate of the newborns were compared between the two groups. Results Compared with the control group, the analgesia effects and cesarean section of the observation group were significantly lower (χ 2=108.400, 8.173; P<0.01); the total delivery time of vaginal delivery was shorter(u=9.169,P<0.01),and the aid-vaginal birth rate was lower(χ 2=3.908,P<0.05).The anoxia rate of newborns in the two groups revealed no significant difference (χ 2=0.198,P>0.05) . Conclusion Doula delivery with inhalation of laughing gas can decrease labor pain, total delivery time and rate of cesarean section. It shows no side effects on both mother and fetus.
