摘  要:目的探讨肾上腺髓质素在低氧性肺动脉高压形成中的作用。方法参与实验全过程的SD大鼠共24只。以右心导管法测定肺动脉压力 ,以放射免疫测定法测定左、右心室血浆中ADM的含量 ,并应用真彩色细胞图文分析系统对大鼠肺动脉进行显微观察。结果低氧处理大鼠后 ,肺动脉压随低氧时间延长而逐渐升高 (P<0 01)。右心室血浆ADM含量随低氧时间的延长逐渐增高 ,缺氧7d、14d、21d组大鼠出肺血ADM含量分别比对照组增加了83.3%、100%、107.1%(P<0.01)。左心室血浆ADM含量与右心室血浆ADM含量的变化有相似的规律。对照组左心室血浆ADM含量较右心室血浆ADM含量增高 ,(P<0.01)。慢性低氧大鼠肺组织形态学指标在显微镜下也有显著的变化。结论低氧后左、右ADM水平的升高可能参与了低氧性肺血管结构重建及低氧性肺动脉高压的形成。Objective To explore the possible role of adrenomedullin(ADM)in the development of hy-poxic pulmonary hypertension.Methods A total of24SD rats were involved in the whole process of the experiment.Pulmonary artery pressure of each rats was evaluated by using a right cardiac catheterization procedure.Plasma concentration of ADM on left/right ventricle was measured with radioimmunoassay.The small pulmonary arteriolaes of the chronic hypoxic rats were examined with light microscopes and morphometrics analysis.Results Pulmonary artery mean pressure was signifi-cantly increased in rats exposed to hypoxia(P<0.01).In control group,the levels of ADM in plasma of LV were significantly higher than that of RV(P<0.01).Compared with control,the conetents of ADM in RV/LV plasma of the hypoxic rats were significantly enhanced along with proloflging the time of exposure to intermittent normobaric hypoxia.In7,14and21day hypoxic groups ADM of RV were increased about40.4%,67.3%and69.2%(P<0.01)and ADM of LV about83.3%,100%and107.1%respectively(P<0.01).The changes of lung morphological indices in the chronic hypoxic rats were great.Conclusion The up-regulation of ADM on left/right vefltricle due to hypoxia is probably involved in the development of hypoxic pulmonary vascular structural remodeling and even hypoxic pulmonary hypertension.
