流行性出血热灭活疫苗对慢乙肝HBV抑制的疗效初步观察

摘　　要：目的观察流行性出血热灭活疫苗对慢乙肝ＨＢＶ抑制的临床疗效。方法选择符合病毒性肝炎防治方案（试行）诊断标准的病例共５００例，其中乙肝病毒携带者治疗组１５７例，慢乙肝治疗组２９３例，乙肝后肝硬化治疗组５０例，并设慢乙肝干扰素治疗对照组３０例。各治疗组均以流行性出血热灭活疫苗肌注，治疗中每周检测血尿常规，每半月测肝、肾功能，每月测乙肝三系（ＥＬＩＳＡ）与ＨＢＶ－ＤＮＡ（ＰＣＲ），并记录疫苗不良反应。结果各治疗组的即刻完全应答率均高于对照组的阴转应答率。慢乙肝疫苗治疗组即刻完全应答率（３４．４％）低于慢乙肝干扰素治疗的对照组（４０．０％）（Ｐ＞０．０５）；慢乙肝疫苗治疗组２年期完全应答率（１５．０％），低于干扰素治疗的对照组（２３．３％）（Ｐ＞０．０５）。结论流行性出血热灭活疫苗作为干扰素诱生剂，可用作慢乙肝病毒携带者，慢乙肝及乙肝肝硬化代偿期患者的转阴治疗，是安全、有效、经济、方便的，值得应用推广。Objective To determine the clinical effects of inactivated vaccine of epidemic hemorrhagic fever in patients with hepatitis B. Methods Altogether 500 cases according to the standard established in conference of viral hepatitis in Shanghai in 1990 were enrolled in the study. The patients were divided into three groups:157cases of HBV carrier,293 cases of chronic hepatitis B and 50 cases of liver cirrhosis caused by hepatitis B. All the three study groups were treated with vaccine of epidemic hemorrhagic fever. As a control group another 50 chronic hepatitis patients were treated with interferon. Blood routine test, urine routine test,liver function,kidney function and HBV-DNA marker were detected monthly.The side effects of the vaccine were observed in detail. Results The rate of complete response in study groups was higher than that in natural sero-convertion after finishing the therapy. The rate of complete response in the study group was lower than that of control group(X]P >0.05). Response rates were 34.4% and 40.0% respectivel. 2 years later,the rate of complete response in study group was lower than that in control group (P >0.05).Response rates were respectively 15.01% and 23.33%. ConclusionsZ]The vaccine therapy is safe,effective,economic and convinient and worthwhile of using.The vaccine of epidemic hemorragic fever,as interferon indacer,can be used to treat HBV carrier,chronic hepatitis B and liver cirrhosis.
