口服负荷量普罗帕酮转复阵发性心房纤颤的临床疗效观察

摘　　要：目的　探讨口服负荷量普罗帕酮转复阵发性心房纤颤 (PAF)的临床疗效和安全性。方法　将 62例房颤持续时间 2～ 48小时的患者随机分为两组 ,治疗组 3 2例口服悦复隆 3 0 0mg～60 0mg ,对照组 3 0例静注西地兰 0 .4～ 0 .8mg。记录从用药到转复为窦性心律的时间。 结果　治疗组转复率 4小时内为 5 6.3 % ( 18/3 2 )、8小时内为 62 .5 % ( 2 0 /3 2 )、12小时内为 71.9% ( 2 3 /3 2 ) ;对照组转复率则分别为 16.7% ( 5 /3 0 )、40 .0 % ( 12 /3 0 )及 46.7% ( 14 /3 0 )。各时间段治疗组PAF转复率均明显高于对照组 (P <0 .0 5 )。 4小时内平均转复时间治疗组明显短于对照组 (P <0 .0 1)。治疗组中未转复成功的 9例患者房颤持续时间明显高于转复成功者 (P <0 .0 1)。结论　对于不伴有心功能不全、缓慢心律失常及急性心肌缺血的非瓣膜病PAF患者 ,口服负荷量普罗帕酮能有效和迅速地使PAF转复 。Objective To study the clinical effect and safeness of oral loading dosage propafenone in conversion of paroxysmal atrial fibillation (PAF).Methods Sixty-two patients with PAF for 2～48 hours were divided into two groups.32 cases in trail group received 300～600mg propafenone per os;30 cases in control group received 0.4～0.8mg cedilanid intravenous injection.The time of sinus rhythm converted when drug was used was recorded.Results The rate of conversion in trail group was 56.3% (18/32) within 4 hours,62.5% (20/32) within 8 hours,71.9% (23/32) within 12 hours;while the rate of conversion in control group was 16.7% (5/30)、40.0% (12/30)、46.7% (14/30),respectively.The rate of conversion in trail group was significantly higher than that in control group in every phase(P<0.05).Average conversion time in trail group was significantly shorter than that in control group in 4 hours phase(P<0.01).The lasting time of PAF of 9 cases in trail group who fail to be converted were significantly longer than that of those who succeed in being converted(P<0.01).Conclulsion Oral loading dosage propafenone was efficient and quick in conversion of PAF of nonvalvular heart disease patients without heart insufficiency、brady-arrhythmia and acute myocardial ischemia and in shortening of the lasting time of PAF.
