螺旋CT动态增强扫描在进展期胃癌诊断及分期中的价值

摘　　要：目的　评价低张力口服水螺旋CT扫描对进展期胃癌的诊断、分期及可切除性判断的价值。方法　 10 9例患者口服水 ,在低张状态下进行动态增强扫描。判断肿瘤诊断及分期 ,预测手术可切除性 ,并与手术及病理相对照。结果　病灶检出率 94.5 %。CT表现为胃壁增厚、软组织肿块、胃壁多层结构破坏 ,淋巴结及邻近、远隔脏器的转移。螺旋CT的T、N及TNM分期准确率分别为 86.7%、68.3 %和 72 .5 %。预测可切除性的准确率 92 .3 %。结论　螺旋CT在进展期胃癌诊断、分期及可切除性判断等方面具较高的价值 。Objective To evaluate the diagnostic and preoperative staging value of spiral CT(SCT) in advanced gastric carcinoma(AGC) with dynamic enhencement and drug-induced hypotonia and water-filling.Methods All 109 cases were dynamic enhencement scanned preoperatively by spiral CT with gastric distension and hypotonia. Diagnosis of AGC was made, and preoperative staging of carcinoma respectability was evaluated. The results were correlated with histological findings. Results Total 103 cases of AGC were detected by SCT (103/109,94.5%). Gastric wall thickening, mass and multi-layered structure disappearance were the characters of AGC. The accuracy of SCT in the T, N and TNM staging was 86.7%, 68.3 % and 72.5%, respectively. The accuracy of respectability was 92.3%. Conclusion SCT is a valuable guidance on the diagnosis and therapy of gastric carcinoma.
