肾动脉栓塞治疗肾出血的临床评价

摘　　要：目的研究创伤性肾出血在保守治疗无效情况下进行肾动脉栓塞的造影表现及栓塞治疗肾出血的方法。方法采用超选择性肾动脉栓塞术治疗31例肾损伤患者,所有病人在栓塞前都进行肾动脉造影证实诊断。栓塞材料以明胶海绵加金属钢圈。结果31例患者栓塞后,临床症状较快改善,1月后行静脉尿路造影检查,伤侧肾功能恢复。仅1例栓塞后仍有持续性血尿行肾切除。结论对创伤性肾出血病人,采用肾动脉栓塞治疗,止血效果好,能最大限度保护肾功能,并发症少,是目前治疗肾损伤出血的一种安全、有效的方法。Objective To evaluate the arteriographic findings and procedures in renal artery embolization of traumatic renal hemorrhage under the conservative treatment to be noneffective condition.Methods Thirty-one patients with traumatic renal hemorrhage were demonstrated by renal arteriography before the embolization.Embolized materials used for the study were gelfoam and steel coils.Results Following renal artery embolization,the clinical symptoms in all patients were quickly improved and intravenous urogrpahy at one month after the embolization showed that the renal function of the traumatic side already recovered,except one case underwent surgical resection of the traumatic kidney because of continued hemaluria after renal artery embolization.Conclusion For the patients with traumatic renal hemorrhage,renal artery embolization has not only a better hemostatic efficacy but can protect as possible as maximum limit the renal function,in same time,the complications related to the procedure is also rare,therefore,it is now a safe,effective means for treating traumatic renal hemorrhage.
