皮瓣移植在四肢火器伤中的应用

摘　　要：自1985年6月～1991年12月应用皮瓣移植修复四肢火器伤所致的软组织缺损18例。其中局部皮瓣转移修复3例,前臂岛状皮瓣修复6例;股前外侧皮瓣修复4例,胸脐皮瓣修复3例,阔筋膜张肌皮瓣修复2例。18例中,急诊修复8例,伤后2周内修复8例,4周内修复2例。皮瓣全部成活,外形及功能恢复满意。提示精细而彻底的清创是火器伤早期行皮瓣移植的先决条件,并应尽可能地在急诊下早期行皮瓣移植,一期修复血管、神经、肌腱及骨骼。During the period of June, 1985～Dec. 1991, 18 cases of injured or incomplete soft tissues causedby firearms were repaired by using skin flap grafting. Of the 18, 3 cases were repaired with partial skinflap removal, 6 forearms with island-shape skin flap, 4 outer front thigh and 3 chests and abdomenswith skin flap repairs, and 2 tensor fascia lata with myocutaneous flap. Among them, 8 personsreceived emergency treatment, 8 were repaired within 2 weeks after being wounded, and 2 within 4weeks. The operations proved successful,and satisfactory recovery was achieved in external forms andfunctions. It is concluded that careful and thorough debridement of the wounds is the prerequisite forskin flap graft, and skin flap graft should be done as early as possible under emergency treatment so asto repair the veins, nerves,muscles and bones at the same time, as well as the injured part.
