血、尿β_2-MG对大剂量顺铂肾毒性的早期诊断价值

摘　　要：目的　探讨血、尿β2 -微球蛋白 (β2 - MG)对大剂量顺铂 (HD- DDP)肾毒性的早期诊断价值。方法　动态检测 5 4例接受 HD- DDP(80～ 1 1 0 mg/ m2 )化疗 76周期患者的血、尿β2 - MG及血清尿素氮 (BUN)、肌酐 (Cr)水平变化。结果　化疗后前 3天 (d1～ 3)血、尿 β2 - MG值逐渐升高 ,第 5天开始逐渐下降 ,其中以尿 β2 - MG异常为主 (P<0 .0 1 )。血清 BU N、Cr异常在化疗后第 5、7、1 4 d明显升高 ,滞后于血、尿β2 - MG异常改变。结论　血、尿β2 - MG,尤其是后者 ,对Objective To probe the early diagnostic value of serum and urine β 2- microglobulin(β 2-MG) in HD-DDP nephrotoxicity.Methods The level changes of serum β 2-MG,BUN,Cr and urine β 2-MG were measured dynamically in 54 cases receiving high-dose DDP(80～100 mg/m 2) chemotherapy during 76 cycles.Results In the early stage of chemotherapy(d 1～3 ),the level of serum and urine β 2-MG increased gradually,and lowered gradually from the fifth day.The level changes of urine β 2-MG was significantly higher than that of serum β 2-MG( P <0.01).The level of serum BUN and Cr increased significantly in the cases of late period of chemotherapy(d 5,d 7,d 14 ).Conclusion The measurement of serum and urine β 2-MG,especially the latter,has an important clinical value for early diagnosis of DDP nephrotoxicity.
