股骨干骨折三种治疗方法的临床探讨

摘　　要：目的 :比较钢板内固定术 ,外固定支架治疗及交锁髓内钉治疗股骨干骨折的优越性。方法 :将 119例股骨干骨折病例随机分为三组 ,A组 5 3例行切开复位钢板内固定术 ;B组 3 5例行外固定支架治疗 ;C组 3 1例行交锁髓内钉治疗。治疗后对其疗效及并发症进行观察 ,比较和分析。结果 :A组骨折愈合优良率 81% ,其并发症 :浅表感染 3例 ,深部感染 1例 ,固定失败 2例 ,迟延愈合 5例。B组骨折愈合优良率73 % ,其并发症 ;浅表感染 6例 ,固定失败 2例 ,迟延愈合 1例。C组骨折愈合优良率 92 % ,其并发症 :浅表感染 1例 ,深部感染 1例。结论 :交锁髓内钉是治疗股骨干骨折较理想的方法。Objective:To evaluate the superiority of internal fixation with steel plate、exopexy and locked intramedullary nail in treatment of fracture of femoral shaft.Methods:119 cases of fracture of femoral shaft was divided randomly into three groups-53 cases in group A with Open reduction and internal fixation with steel plate, 35 cases in group B with exopexy and 31 cases in group C with locked intramedullary nail. The effectiveness and postoperative complications were carefully analyzed.Results:The satisfaction rate of union in group A reached 81%. Its complications were superficail infection in 3 cases, deep part infction in 1 cases, fixative fail in 2 cases and delayed healing in 5 cases. The satisfaction rate of union in gourp B reached 73%. Its complications were supericial infection in 6 cases, fixative fail in 2 case and delayed healing in 1 case. The ssatisfaction rate of union in group C reached 92%. Its complications were superficial infection in 1 case and deep part infection in 1 case. Conclusion:The ideal method was locked intramedullary nail in treatment of fracture of femoral shaft.
