VCAM-1在妊娠期高血压疾病患者血清和子宫胎盘床的表达及其与IL-6的关系

摘　　要：目的　探讨妊娠期高血压疾病患者血清血管细胞黏附分子 1(VCAM 1)和白细胞介素 6 (IL 6 )水平的变化 ,二者的关系和VCAM 1在子宫胎盘床的表达 ,及其与妊娠期高血压疾病发病机制的关系。方法　 (1)于2 0 0 0年 3月至 2 0 0 3年 2月 ,青岛大学附属医院采用双抗体夹心酶联免疫吸附法 (ELISA)测定了 6 0例妊娠期高血压疾病患者 (其中妊娠期高血压 2 0例、子疒间 前期轻度及重度各 2 0例 ) ,35例正常晚期妊娠妇女血清VCAM 1和IL 6水平。 (2 )采用免疫组织化学染色法检测了 6 0例妊娠期高血压疾病患者、30例正常妊娠妇女子宫胎盘床螺旋动脉VCAM 1的表达。结果　 (1)妊娠期高血压疾病组血清VCAM 1水平明显高于正常妊娠组 (P <0 0 5 )。 (2 )子宫胎盘床螺旋动脉滋养细胞VCAM 1表达 ,妊娠期高血压疾病组阳性表达率较正常妊娠组明显降低 (P <0 0 5 )。 (3)妊娠期高血压疾病组血清IL 6含量明显高于正常妊娠组 (P <0 0 5 ) ,VCAM 1与IL 6水平呈明显正相关 (r =0 6 3,P <0 0 1)。结论　妊娠期高血压疾病患者血清中VCAM 1和IL 6水平明显升高 ,而子宫胎盘床VCAM 1表达减少。VCAM 1和IL 6参与了妊娠期高血压疾病的发病机制。二者在子疒间 前期患者血清中水平呈正相关。Objective To study the changes of serum VCAM-1 and the expression of VCAM-1 in utero-pl acental bed in hypertensive disorder complicating pregnancy(HDCP) and their rela tionship with IL-6. Methods 60 women with HDCP (including 20 women with gestational hypertension,20 women wi th mild pre-eclampsia,20 women with severe pre-eclampsia) and 35 normal pregna nt w omen were studied.Serum levels of VCAM-1 were measured by Elisa.The expression of VCAM-1 in spiral arteries of utero-placental bed in 60 HDCP and 30 normal p regnant women were evaluated by immunohistochemistry. Results (1)Serum concentration of VCAM-1 in HDCP was higher than that in normal pregn ant group(P<0\^05).(2)The expression of VCAM-1 in cytotrophoblast of spiral arteries in normal pregnant group were significantly higher than that in HDCP g roup(P<0\^05).(3)Serum levels of IL-6 in HDCP group were higher than that i n normal group(P<0\^05).In pre-ecl ampsia group,serum concentrations of VCAM-1 correlated positively with the leve ls of IL-6,r=0.63,P<0\^01. Conclusion The increased levels of serum VCAM-1 and IL-6 may participate in the proces s of vascular endothelium damages in HDCP.The reduced expression of VCAM-1 in u tero-placental bed may relate to superficial implantation.VCAM-1 and IL-6 m ay play a role in the pathogenesis of HDCP.
