骶髂关节平片检查位置的正常X线解剖研究

摘　　要：目的 :探讨骶髂关节解剖特点与X线表现。 方法 :观察骨骼标本 ,对骨骼标本进行多位置投照 ,并将其影像与骨盆平片影像进行研究分析。 结果 :骶髂关节耳状关节面呈短靴形态。骶髂关节影像形如长梭状 ,长梭状影的外缘线代表骶髂关节的前间隙 ,由髂在外骶在内的的关节面前缘构成 ,外线犹如倒置的短靴 ,其最突出部的靴根代表上下关节的分界。长梭状影的内缘代表骶髂关节的后间隙 ,由髂在外骶在内的关节面后缘构成。内外两线汇合处是骶髂关节滑膜部与韧带部的分界点 ,也可把髂骨与骶骨两者在上部相重形成宽大V状间隙的V尖作为分界点。 结论 :对骶髂关节在前后位形成短靴与长梭状形态的构成认识有了明显提高 ,对今后骶髂关节病变的诊断具有一定的指导意义。Objective: To study anatomical feature and X-ray performance of cacroiliac joint. Methods: The skeletal specimen were carefully observed and then projected in multiposition. A comparative study was made by a comparison between the skeletal specimen film and the pelvis plain one. Results: Auriform joint surface of cacroiliac joint presents short boot form. Cacroiliac joint image resembles long shuttle whose outer line which is constituted by the front edge of joint surface whose outside is ilium and inside is sacrum represents the front space of cacroiliac joint. The outer line seems to be an inverted short boot whose the most prominent boot end represents the demarcation between the upper and lower joint. The long shuttle inner line which is constituted by the posterior edge of joint surface whose outside is ilium and inside is sacrum represents the posterior space of cacroiliac joint. Convergence from both outer line and inner line forms a demarcation point between the membranous part and the ligament part of cacroiliac joint. In addition, V apex of lenient V-like space which is formed by overlapping sacrum and ilium is also regarded as another demarcation point. Convulsion: We achieve further knowledge that cacrailiac joint presents short boot and long shuttle image in A-P position. Meanwhile, it is helpful for us to diagnose the cacroiliac joint lesion
