摘　　要：目的探讨酸化血清溶血(Ham’s)试验阳性的自身免疫性溶血性贫血(AIHA)的实验室诊断。方法对2例AIHA的临床及实验室资料进行了回顾性分析。溶血方面的检查采用经典Ham’s试验、直接抗人球蛋白试验(Coombs)和冷凝集素试验(CAT)等。结果2例患者均有黄疸、贫血,网织红细胞比例增高,Ham’s试验均阳性,Coombs试验均表现为C3阳性;CAT显示4℃时效价均明显增高,37℃时仍为阳性,说明有高滴度和宽热幅的异常冷凝集素。结论对于临床上考虑为AIHA的患者,若Ham’s试验阳性,应考虑到宽热幅异常冷凝集素的存在。Objective To explore the laboratory diagnosis of autoimmune hemolytic anemia (AIHA) with positive Ham’s test. Methods The clinical and laboratory data of 2 case of AIHA were studied retrospectively. Ham’s test, direct antihuman globulin test (Coombs’s test) and cold agglutinin test (CAT) were used in the laboratory diagnosis. Results Both 2 cases had jaundice and anemia. Laboratory data showed that reticular red blood cell increased, Ham’s test was positive, Coombs test showed positive C3, and CAT revealed cold agglutinin with high thermal amplitude. Conclusion If Ham’s test is positive in AIHA patients, the presence of cold agglutinin with high thermal amplitude should be considered.
