摘  要:目的 观察窒息新生儿免疫功能及 β 内啡肽 (β EP)的动态变化。 方法  2 0 0 1年 10月至 2 0 0 2年 10月选择广东省肇庆市第一医院儿科住院窒息新生儿 81例和同期健康新生儿 2 0例。窒息儿随机分成两组 :常规治疗组 4 1例和纳络酮治疗组 4 0例。均于生后第 1和第 7天两次采血 ,分别检测 β EP、IL 2、TNF α、CD3、CD4、CD8、IgA、IgM、IgG及CH50 等各项指标。结果 生后第 1天与对照组相比窒息组新生儿免疫功能明显抑制 ,且与窒息程度成正比 ,而 β EP则显著升高 ;生后第 7天 ,常规治疗组与对照组比较 ,β EP、IL 2、TNF α、CD3、CD4 /CD8等项仍有显著差异 (P <0 0 5 ) ,而纳络酮治疗组和常规治疗组比较 ,则 β EP、IL 2、TNF α、CD3、CD8、CD4 /CD8等项有显著差异 (P <0 0 5 ) ;除CD8外 ,常规治疗组 β EP与各免疫指标两次检测差值的相关分析均有显著相关性 (P <0 0 5 )。结论 窒息新生儿免疫功能明显抑制 ,且与窒息程度成正比 ,βObjective To observe the dynamic changes of immunologic function (including immunoglobulin,T-cell subpopulation,Il-2,TNF-α).Methods The asphyxia group contained 81 cases and control group 20 cases.The asphyxia group was divided into two sub-groups :routine treatment group (41cases) and naloxone treatment group (40cases).The patients of routine treatment group were treated as suggested by Han Yukun etc.The patients in the naloxone group were treated with naloxone for a week in addition to the routine treatment.Two blood samples were collected in each case,once on the first day after birth,the other on the seventh day after birth,β-EP、IL-2、TNF-α、CD3、CD4 、CD8、IgA、IgM、IgG and CH 50 were amaligied.Statistical method :statistical analysis including t-test,variance analysis,linear correlation analysis was performed with the statistical program package SPSS for Windows 10 0.P<0 05 was considered significant.Results Compared with the control group,the immunologic function of the asphyxia group was inhibited significantly.CD3、CD4 、CD4/CD8、IL-2、IgA、IgM、IgG were all significantly decreased,CH 50 did not show obvious changes,and β-EP and TNF-α were significantly increased.And the degree of inhibition was positively correlated to the degree of asphyxia.The result of the second detection showed:compared with control group,β-EP、IL-2、TNF-α、CD3、CD4/CD8 still showed significant differences in the routine treatment group (P<0 05).Compared with the routine treatment group,β-EP、TNF-α、CD8 showed a significant decrease and IL-2、CD3 、CD4/CD8 showed a significant increase in the naloxone treatment group (P<0 05).In the routine treatment group,the relative analysis of the balance of β-EP and each immunologic index of twice detection showed:except CD8,the correlation was significant.Conclusion Immunologic function of asphyxia newborns is inhibited markedly and the degree of inhibition is positively correlated to the degree of asphyxia.β-EP participates in inhibitory
