不同镇痛方法对重度妊娠高血压综合征剖宫产病人术后血浆内皮素水平的影响

摘　　要：目的　探讨不同镇痛方法对重度妊娠高血压综合征剖宫产病人血浆内皮素水平的影响。方法　将 6 0例重度妊娠高血压综合征剖宫产病人按术后镇痛方法不同分为PCEA组 (病人自控硬膜外镇痛 )、PCIA组 (病人自控静脉镇痛 )及凯扶兰组 (病人口服凯扶兰镇痛 ) ,每组 2 0例。分别于麻醉前及术后 6h、12h、2 4h、4 8h、72h抽取静脉血 ,测定各组病人不同时段的内皮素水平变化 ,同时测量记录病人的血压变化情况 ,并进行比较。结果　PCEA组及PCIA组术后血浆内皮素水平降低速度明显快于凯扶兰组 (P <0 .0 1) ,且PCEA组的降低更为明显。结论　PCEA镇痛法更有利于妊娠高血压综合征剖宫产病人血浆内皮素水平的降低。Objective To exlplore the effects of different analgetic methods on the postoperative endothelin(ET) in the plasma of patients with severe pregnacy-induced hypertension (PTH).Methods 60 cases of severe PIH were classified into the group of patient's controlling epidural analgesia(PCEA),the group of patient's controlling intravenous analgesia(PCIA) and the kaflan group,with each group having 20 cases.The venous blood was sampled 6 hours,12 hours,24 hours,48 hours and 72 hours before the anesthesia and after the operations respectively.The levels of ET were determined and the changes in blood pressure were also recorded.Results The contents of ET in the groups of PCEA and PCIA were reduced faster the group of Kaflan.And the levels of ET in the PCEA group declined most obviously.Conclusion PCEA can be used to effectively reduce the level of ET in plasma of patients with severe PIH.
