血液稀释对冠心病患者手术QT离散度的影响

摘　　要：目的　观察血液稀释对冠心病患者手术 QT离散度 (QTd)的影响并探讨其意义。方法　选择 6 0例冠心病患者随机分成对照组 ( 组 )和血液稀释组 ( 组 )。 组在术前采集自体血 2 0 0～ 6 0 0 m l,同时输注平衡液使血液达到中度稀释状态 ; 组未采取以上措施。两组均于术前、术中 1小时、术后第 1、 2天记录同步 12导联心电图 ,测量 QTd并观察室性心律失常。结果　两组 QTd术中、术后与术前相比均有所增加 (P<0 .0 5或 P<0 .0 1) ,但 组增加明显多于 组 (P<0 .0 1) , 组室性心律失常也较 组容易发生。结论　血液稀释可相对缩短 QTd,减少室性心律失常发生。ObjectiveTo observe the change and significance of QT dispersion(QTd) in patients with cornary heart disease.Methods Sixty patients were divided randomly into two groups as control group(Ⅰ) and hemodilution group(Ⅱ). 200~ 600 ml of every patient's blood was collected via internal jugular vein as to underlie the middle degree of hemodilution before operation in group Ⅱ.Electrocardiograms were recorded before operation,during one hour of operation,and at first,second day after operation.QTd was measured and cardiac arrhythmia was observed for two groups.Results The QTd of two groups were increased after operation(P<0.05 or P<0.01),but the QTd of group Ⅰ increased more than that of group Ⅱ(P<0.01),and in group Ⅰ ventricular arrhythmia was easier to happen.Conclusion Hemodilution can shorten QTd and reduce ventricular arrhythmia.
