冠心病患者血浆血管性假血友病因子及其前肽水平与血管内皮损伤的关系

摘　　要：目的:内皮细胞损伤而致的功能障碍,是动脉粥样硬化的始动环节,而血管性假血友病因子前肽(vonWillebrandfactorpropeptide,vWF-pp)及成熟血管假性血友病因子(vonWillebrandfactor,vWF)在不同侧面反映内皮细胞的损伤。探讨冠心病患者血浆血管性假血友病因子前肽及成熟vWF的水平具有重要的临床意义。方法:采用酶联免疫吸附分析(enzymelinkedimmuno-sorbentassay,ELISA)双抗体夹心法对48例泰安市中心医院临床确诊的病程在一周内的急性冠心病患者,50例无心血管疾病、糖尿病、高血压等疾病的年龄,性别与冠心病组一致的体检者,且标本无溶血,脂血者血浆vWF-pp,vWF进行测定。结果:冠心病组vWF-pp水平为(1098.83±331.91)μg/L,与同年龄段对照人群vWF-pp水平(502.86±119.49)μg/L比较差异有显著性意义(P<0.01)。冠心病组vWF水平为(21.25±13.72)mg/L,而对照人群vWF水平为(11.13±7.02)mg/L,两者间差异有非常显著的意义(P<0.01)。对冠心病组的相关性分析显示,vWF与vWF-pp之间具有显著的正相关关系(r=0.242,P=0.03)。结论:冠心病组患者血浆vWF-pp和vWF增高,且以vWF-pp增高更为明显。vWF-pp和vWF的同时检测对于尽早的发现或及时的处理急性冠脉事件具有重要意义。AIM: Vascular dysfunction induced by endothelia cell damage is the start for atherosclerosis. The differential response of von Willebrand factor (vWF) and propeptide (vWF pp) level reflect the endothelia cell damage. The aim of this paper is to investigate the levels of von Willebrand factor (vWF) and vWF propeptide(vWF pp) in patients with coronary heart disease(CHD), and evaluate their clinical significance.METHODS: Forty eight patients with acute CHD, who were clinically diagnosed in Taian Central Hospital within one week of course, and 50 subjects without cardiovascular disease, diabetes and hypertension, whose age and gender were coincident with the CHD patients, were involved in this study. The levels of vWF and vWF pp were determined by Sandwich enzyme linked immuno sorbent assay(ELISA) in both groups. RESULTS: The level of vWF pp had a significant difference between the CHD group[(1 098.83±331.91)μg/L] and the control group[(502.86±119.49) μg/L] (P< 0.01). There was very significant difference in the level of vWF between the CHD group [(21.25±13.72)mg/L] and the control group [(11.13±7.02) mg/L](P< 0.01). The results of correlation analysis showed that there was significant positive correlation between vWF and vWF pp in the CHD group (r=0.242,P=0.03).CONCLUSION: In the CHD patients, the plasma levels of vWF pp and vWF are increased, especially the level of vWF pp.The determinations for vWF pp and vWF have importance significance in early detection and prompt the treatment of acute coronary incidents.
