胃肠术后1号对急性腹膜炎患者血中炎性介质及内毒素水平的影响

摘　　要：目的 :了解促炎因子在急性弥漫性腹膜炎患者血中的动态变化 ,并探讨胃肠术后 1号的治疗作用。 方法 :采用鲎试剂基质偶氮显色定量法及酶联免疫吸附法 ,对 2 0例患者 (随机分为两组 :常规治疗组 10例 ,胃肠术后 1号加常规治疗组10例 )测定血浆内毒素、血清促炎因子 (TNFα、IL6 )水平。 结果 :急性弥漫性腹膜炎患者促炎因子 (TNFα、IL6 )呈现不同程度的升高 ,经治疗随病情好转 ,内毒素及炎症介质呈现不同程度的下降 ,而胃肠术后 1号加常规治疗组内毒素及TNFα、IL6下降幅度显著 ,优于常规治疗组 (P <0 0 5 )。 结论 :内毒素、促炎因子在急性弥漫性腹膜炎患者中起重要作用 ,可作为评估急性弥漫性腹膜炎患者严重程度的指标 ,胃肠术后l号使内毒素、TNFα、IL6水平下降 。Objective To study the changes of serum proinflammatory cytokines in patients with acute diffuse peritonitis and to explore the effect of Chinese medicinal herbs Methods Twenty patients with acute diffuse peritonitis were randomly divided into 2 groups,10 in the Wei Chang Shu Hou 1 group and 10 in the control group.Plasma endotoxin (lipopolysaccharide,LPS)serum proinflammatory cytokines,including tumor necrosis factor α(TNFα) and interleukin 6 (IL-6) by using limulus reagent stroma azo colored quantitative method and enzyme linked immunosorbebt assay(ELASA).Results The levels of LPS increased at the early stage of acute diffuse peritonitis,the levels of proinflammatory cytokines were elevated in various degrees.The levels of LPS and inflammatory cytokines reduced to some extent in accord with the improvement of condition after treatment,the levels of LPS,TNFα IL-6 decreased more significantly in the Wei Chang Shu Hou 1 group than those in the control group( P <0 05). Conclusion LPS and proinflammatory cytokines play important roles in acute peritonitis process.The Chinese herbal medicine Wei Chang Shu Hou 1 can reduce the blood levels of LPS,TNFα and IL-6.
