摘  要:目的 探讨司维拉姆 (Renagel)对高磷饲料慢性肾衰竭 (CRF)大鼠高磷血症、甲状旁腺细胞增生的干预作用。方法 将 18只大鼠随机分为 3组 :假手术组、5 /6肾大部切除组 (CRF组 )、CRF +Renagel 治疗组 (Renagel 组 )。所有大鼠均喂饲高磷饲料 (P1 2 % ) ,治疗组饲料中含 0 2 %Renagel 。术后第 2、7、14d测定血磷 (P)、血钙 (Ca)、血肌酐 (Cr)、甲状旁腺激素 (PTH) ,第 14d检测甲状旁腺核增殖抗原 (PCNA)。结果 血磷CRF组最高 ,治疗组最低 (P <0 0 5 ) ;iPTH治疗组明显低于CRF组 (P <0 0 5 )。Renagel 组PCNA阳性率 (2 7± 6 ) %显著低于CRF组 (33± 7) % (P <0 0 5 ) ,与假手术组 (2 6± 5 ) %无显著差异 (P >0 0 5 )。结论 Renagel 能快速 (2d)降低早期CRF大鼠的高血磷、高PTH ,防治甲状旁腺增生。Objective To study the effect of sevelamer hydrochloride(Renagel )on inhibiting the parathyroid gland hyperplasia in chronic renal failure rats on high phosphate diet.Methods 18 eight-week-old male Sprague-Dawley rats were selected and divided into three groups at random.6 rats served as normal control group.The remaining rats were given 5/6 subtotal nephrectomy to establish the model of CRF.One was the model control group,the other was the dietary sevelamer treatment group.All of rats were on a diet with 1.2% phosphate.Rats in treatment group were on a diet containing 2% sevelamer.Blood was taken for phosphorus,calcium,creatinine and intact PTH determinations at the following intervals:2 day,7 day,and 14day.Parathyroid glands were removed for immunohistochemistry of PCNA at 14 th day.Results Serum P and PTH levels in CRF rats were significantly higher than normal control and sevelamer treatment rats(P<0.05).Nuclear proliferation Indexes of parathyroid in sevelamer-treated rats were lower than that in CRF rats(P<0.05).Conclusion Sevelamer treatment arrested hyperphosphatemia and PTG hyperplasia accompanied by the elevation of serum PTH levels.
