缺氧性肺动脉高压患者血管紧张素及其转化酶的改变和巯甲丙脯酸疗效的观察

摘  要:测定28例缺氧性肺动脉高压患者和23例正常人的血管紧张素Ⅰ、Ⅱ(ATⅠ及ATⅡ)及血管紧张素转化酶(ACE),并对其中8例患者进行了巯甲丙脯酸治疗试验。结果显示缺氧性肺动脉高压患者与正常对照组相比,其血浆ATⅡ水平明显升高,血清ACE活性明显降低;常规疗法对降低患者的ATⅡ水平和平均肺动脉压(mPAP)的作用不大,而加用巯甲丙脯酸后可使ACE活性进一步降低,从而明显降低ATⅡ和mPAP。由此表明ATⅡ在缺氧性肺动脉高压的发病中起着重要的作用,而巯甲丙脯酸则可能是一种治疗该症的有效药物。The levels of angiotensin Ⅰ,Ⅱ (ATⅠ, Ⅱ)and angiotensin convertingen-zyme (ACE) were measured in 28 patie ntswith hypoxic pulmonary hypertensionand 23 normal individuals. The effects ofcaptopril were observed in 8 of 28 patients.The results showed that the average levelof ATⅡ in the patients was significantlyhigher than that in normal control (P<0.05), but the average level of ACE in thein the controls (P<0.05). In 8 patients,after the use of captopril, an ACE inhibi-tor, for two weeks, the levels of ATⅡ,ACE and mean pulmonary arterial pres-sure (mPAP)were markedly decreased (P<0.05). But the other 20 patients treatedwith antibiotics, antiasthmatics, diureticsand oxygen therapy for two weeks did notshow any alterations in the levels of ATⅡ,ACE and mPAP.
