摘　　要：目的　评价单次激发半傅立叶FSE序列MRCP图像质量及诊断价值 ,分析胆道梗阻性疾病的MRCP影像学表现。方法　 2 18例病人行单次激发半傅立叶FSE序列MRCP扫描 ,然后对图像进行质量评价 ,根据ERCP及手术结果对照分析胆道梗阻性疾病的MRCP的影像学征象和特点 ,讨论单次激发半傅立叶FSE序列MRCP在胆道梗阻性疾病中的诊断价值。结果　单次激发半傅立叶FSE序列MRCP扫描时间为 5～ 7s,优良图像为 82 5 % ,合格图像为 10 6 % ,差为 6 9%。单次激发半傅立叶FSE序列MRCP在胆道梗阻性疾病中 ,对各类梗阻疾病的定位诊断率为 10 0 % ,梗阻端形态 ,胆胰管之间的关系均可显示。病因定性诊断率为 95 5 %。结论　单次激发半傅立叶FSE序列MRCP不受呼吸运动影响 ,图像优质率高 ,扫描速度快 ,可显示各种疾病的胆胰管梗阻改变 ,对诊断较小的结石及泥沙样结石有较大的价值 ,诊断较早期胆管癌准确率高 ,可满足临床诊断的要求。Objective To evaluate the image quality and value for clinical diagnosis of single-shot half-Fourier FSE sequence MRCP in bile duct obstructive diseases. Methods After the single-shot half-Fourier FSE sequence MRCP was performed in 218 patients, the image quality and image signs of bile duct obstructive diseases were analyzed according to the operation or ERCP or follow-up results to discuss the value of the technique in diagnosing different bile duct obstructive diseases. Results The time of scanning with the single-shot half-Fourier FSE sequence MRCP was 5-7 seconds. The excellent and good images accounted for 82.5%, the fair ones for 10.6% and poor ones for 6.9%. Using the technique, the diagnostic accuracy for the bile duct obstructive diseases was 100%. Meanwhile, the shape of the obstructive edge and relation of the bile duct to pancreatic duct could also be revealed by the technique. The diagnostic accuracy of the technique for the bile duct obstructive nature was 95.5%. Conclusions The single-shot half-Fourier sequence MRCP can not be affected by respiratory motion during examination. The rate of excellent and good images is high;the scanning time is less for the bile duct obstructive diseases. Both the obstructive level and nature can be diagnosed by the technique. Meanwhile, it can show the obstructive changes of the bile duct and pancreatic duct and is of high value in diagnosing small and sand-like stones and judging early bile duct tumor.
