超声心动图在心尖肥厚型心肌病诊断中的临床应用

摘　　要：目的　探讨超声心动图在评价心尖肥厚型心肌病 (AHCM)中的作用。方法　 12例 AHCM患者和 2 1例高血压性心脏病患者均行超声心动图和心电图检查 ,AHCM患者中 6例行冠状动脉造影及左室造影 ,3例行核磁共振检查。结果　超声心动图显示 AHCM组心尖部心肌肥厚 ,与高心病组有显著差异 [(19.3± 4 .8) mm vs(9.2± 3.1) mm,P<0 .0 5 ],心尖部心腔狭窄 ;心电图表现为巨大倒置 T波 ,R波振幅高 ;冠脉造影为阴性。结论　超声心动图在诊断 AHCM中是一种不可缺少的方法。Objective To evaluate echocardiography in diagnosis of apical hypertrophic cardiomyopathy(AHCM).Methods A total of 12 patients with AHCM and 21 patients with hypertension were examined with echocardiography and ECG,6 ptients with AHCM underwent coronary angiography and 3 patients undrwent MRI.Results In AHCM group,Left ventricular apex thickness were significantly thicker than that of the hypertention group(193±48mm vs 92±31mm,P<005)The apical cardiac chamber became so small in systolic phase,but the results of coronary angiography were normalECG showed giant T wave and increase of R wave. Conclusions Combined with other methods,echocardiography is valuable in diagnosis of AHCM.
