尿视黄醇结合蛋白与尿微量白蛋白在慢性肺心病患者肾损害中的临床意义

摘　　要：目的探讨尿视黄醇结合蛋白 (uRBP)与尿微量白蛋白 (umAlb)在慢性肺心病患者肾损害中的临床价值。方法 6 5例慢性肺心病患者按传统肾损害标准分成无肾损害组和有肾损害组 ,另取 30例健康人为对照组。各组均行uRBP、umAlb、血尿素氮 (BUN)、肌酐 (Scr)测定 ,并行对比分析。结果慢性肺心病有肾损害组uRBP、umAlb含量明显高于对照组 (P <0 0 1) ,慢性肺心病有肾损害组较无肾损害组uRBP、umAlb含量明显升高 (P <0 0 1)。而慢性肺心病无肾损害组uRBP、umAlb含量较对照组也明显升高 (P <0 0 1)。结论uRBP与umAlb可作为慢性肺心病肾损害早期诊断的敏感指标 ,对慢性肺心病的治疗和预防慢性肺心病肾功能损害的发生、发展可能有重要临床意义。Objective To explore the clinic value of urine retinal-binding-protein (uRBP) and urine minim albumin (umAlb) in patients with kidney damage chronic cot pulmonale.Methods 65 cases of chronic cot pulmonale were devided into non-kidney damage group and kidney damage group by the traditional kidney damage criterion and another 30 healthy beings were selected as the contrasting group, uRBP, umAlb,blood urea nitrogen and serum creatinine of all the objects were detemined, and analysed comparatively between groups. Results The levels of uRBP and umAlb in chronic cor pulmonale with kidney damage group were much higher than that in the control group (p<0.01) , The levels of uRBP and umAlb in chronic corpulmonale with kidney damage group were also much higher than in the non-kidney damage group, while the levels of uRBP and umAlb in chronic cot pulmonale with non-kidney damage group were much higher than in the contrasting group(p<0.01).Conclusions The uRBP and umAlb can be considered as the sensitive index in the diagnose of chronic cor pulmonale with kidney damage, meanwhile they have significant diffirence in the treatment, improvement of prognosis and prevention of chronic corpulmonale bom happening and developing.
