全视网膜光凝术后角膜知觉的改变

摘　　要：目的探讨糖尿病眼底病变、视网膜中央静脉阻塞病人在全视网膜光凝术光凝后的角膜知觉改变。方法 28眼糖尿病眼底病变病人、16眼视网膜中央静脉阻塞病人在应用倍频ND:YAG激光行全视网膜光凝,光凝斑大小:200～300um,光凝时间:0.2～0.4秒,光凝斑强度以达到中等强的为标准。采用Cochet-Bonnnet角膜知觉测量仪测量学凝前及光凝后1周、2周、3周、4周、5周、6周及2月的角膜知觉。结果糖尿病眼底病变组术前的角膜知觉为2.29cm,术后1周、2周、3周、4周、5周、6周及2月的角膜知觉分别为1.40cm、0.64cm、1.59cm、1.92cm、1.72cm、2.20cm、2.24cm。与术前相比,术后1～5周的角膜知觉明显减退(配对t检验,P<0.05)。视网膜中央静脉阻塞组术前的角膜知觉为3.65cm,术后1周、2周、3周、4周、5周、6周及2月的角膜知觉分别为1.25cm、1.38cm、1.65cm、1.80cm、2.37cm、3.28cm、3.78cm,与术前相比,术后1～5周的角膜知觉明显减退(配对t检验,P<0.05)。在糖尿病组的角膜知觉减退值与视网膜中央静脉阻塞组相比,二者在光凝后1～5周存在有明显统计学差异(t检验,P<0.05)。结论糖尿病眼底病变及视网膜中央静脉阻塞行全视网膜光凝后的角膜知觉有明显减退,在光凝后6周恢复至术前水平。Purpose To evaluate and compare the recovery postoperative corneal sensitivity after retinal coagulation in diabetic retinopathy and central retinal vein occlusion.Methods 28 consecutive eyes with diabetic retinopathy and 16 consecutive eyes with retinal vein occlusion un- derwent panretinal photocoagulation using an ND:YAG laser.The laser was set at a spot size of 200～300um,a duration of 0.2～0.4 seconds and power necessay to achieve a moderate intensity bum.The panretinal photocoagulation was completed in 2～4 times by 1 week interval.Corneal sen- sitivity was tested preoperatively and 1,2,3,4,5,6,weeks and 2 months postoperatively using the Cochet-Bonnnet esthesiometer.Results The value of corneal sensitivity was 2.29cm prcoperativity,1.40cm,1.64cm,1.59cm,1.92cm,1.72cm,2.20cm,2.24cm of the time intervals of 1,2,3,4,5,6,weeks and 2 months postoperativity in diabetic group respectively,and the cmeal sesitivity was 1.25cm, 1.38cm,1.65cm,1.80cm,2.37cm,3.28cm,3.78cm during the follow-up periods in central retinal vein occlusion group,it was reduced during the first 5 weeks after panretinal photocoagulation in the two groups.Conclusions The corneal sensitivity is reduced after panretinal photo- coaguation in patiens with diabetic retinopathy and central vein occlusion.
