绝经后妇女压力性尿失禁的临床治疗研究

摘　　要：目的 :探讨治疗绝经后妇女压力性尿失禁的非手术治疗的效果。方法 :以配对研究方法分析使用阴道雌三醇软膏配合盆底肌肉锻炼、单用阴道雌三醇软膏、安慰剂配合盆底肌肉锻炼三种治疗方法治疗绝经后妇女压力性尿失禁的疗效。结果 :单用阴道雌三醇软膏组漏尿症状减轻仅 16 .6 % ,治疗前后尿失禁量减少的差异无显著性 (P >0 .0 5 ) ;安慰剂配合盆底肌肉锻炼组漏尿症状减轻有效率 6 3.3% ,治疗前后尿失禁量明显减少 (P <0 .0 0 1) ;使用阴道雌三醇软膏配合盆底肌肉锻炼组漏尿症状减轻 86 .7% ,治疗前后尿失禁量明显减少 (P <0 .0 0 1)。结论 :阴道使用雌三醇软膏配合盆底肌肉锻炼治疗起到协同作用 ,治疗绝经后妇女压力性尿失禁疗效明显 ,方法简单、安全 。Objective: To investigate therapeutic effect of nonsurgical therapies for postmenopausal women's stress incontinence. Methods: In matched survey method, we analyzed therapeutic effects of viginal estriol ointment combined with pelvic diaphragm muscle exercise, estriol monotherapy and placebo combined with pelvic diaphragm muscle exercise for postmenopausal wonen's stress incontinence.Results: In vaginal estriol ointment monotherapy group, relief rate of leakage urine was only 16.6%. and there was no significant difference between pre and post treatment for dicrease in volume of incontinence(P>0.05). In placebo combined with pelvic diaphragm muscle exercise group, effective rate for leakage urine was 63.6%, and the volume of incontinence decreased obviously after treament(P<0.001). In viginal estriol ointment combined with pelvic diaphragm muscle exercise group, relief rate of leakage urine was 86.7% and the volume of incontinence decreased greatly after treatment (P<0.001). Conclusion: Vaginal estriol ointment combined with pelvic diaphragm muscle exercise had a marked therapeutic effect for postmenopausal women's stress incontinence, and was proved to be a simple and safety method, so it is worthy to popularize in clinic.
