摘  要:目的:为了研究Fas/FasL介导的细胞凋亡在桥本氏甲状腺炎发病机制中的作用,评价其在治疗中的意义.方法:采用流式细胞仪、双抗体夹心ELISA法及免疫组化方法分别测定桥本氏甲状腺炎(HT)及正常时照组外周血T淋巴细胞Fas抗原表达情况,血浆中sFas水平及甲状腺腺泡细胞Fas抗原及Fas配体的表达情况.结果:发现HT组甲状腺细胞Fas和FasL表达强度显著高于正常对照组(P<0.01),血浆sFas水平(2.05ng/ml±0.54ng/ml)明显低于正常组(3.59ng/ml±0.68ng/ml,P<0.001),HT外周血T淋巴细胞Fas抗原阳性细胞数(79.99%±16.08%)显著高于正常对照组(57.22%±11.57%,P<0.001).结论:Fas系统介导的细胞凋亡与桥本氏甲状腺炎发病密切相关。Objective: To investigation the role of apoptosis induced by Fas/FasL in the pathogenesis of Hashimoto's thyroiditis and to evaluate its significance in pathogenic course and treatment. Methods: The anticoagulant blood samples from 24 cases of HT and 35 cases of healthy persons were analyzed for the expression of Fas antigen in T lymphocytes and for the plasma level of soluble Fas separately by flow cytometry assay and enzyme linked immunosorbent assay (ELISA). Meanwhile, an immunohistochemical technique was performed for detecting Fas antigen and FasL expression in the thyroid adenocytes from 20 additional pathologic samples, including 10 cases of HT and healthy persons respectively. Results: The Fas and FasL immunohistochemical stains in HT were positive and stronger than those of normai controls (P<0.01). Compared with normai controls (3,57±0.68ng/ml), the mean level of Fas in HT (2.05±0.54ng/ml) was significantly lovver (P<0.01). The expression of Fas of periphearl T lymphocytes in HT (79,99±165.08%) was stronger than that of normai controls (57.22±11.57%, P<0.001). Conclusion: Fas antigen-mediated apoptosis might be closely related to the pathogenesis of Hashimoto's thyroiditis.
