术中彩色多普勒超声检查在原位肝移植术中的应用

摘　　要：目的　为确保原位肝脏移植手术后移植肝的血流量 ,减少由于手术造成的血管并发症 ,采取肝移植手术中进行彩色多普勒超声检查。方法　选用高频手术专用探头 ,于手术中门静脉、肝动脉吻合完成后立即测量肝动脉及门静脉血流速度 ,并计算快速充盈时间及阻力指数。结果　 (1 )彩色多普勒超声可直接观察到移植肝门静脉、肝动脉血流灌注情况 ;(2 )肝动脉峰值流速不得低于 2 5 cm/s,门静脉平均流速不得低于 (45± 1 6 .5 ) cm/s;(3)肝动脉加速度时间应 <0 .0 7s,阻力指数 >0 .5 6 ;(4)可对照结扎分流静脉前后门静脉血流速度的变化。结论　术中彩色多普勒超声检查是确保移植肝血流灌注的最有效手段。Objective Intraoperative color Doppler ultrasonography was used during orthotopic liver transplantation surgery to ensure satisfactory blood flow of the postoperative graft and to reduce the incidence of complications causing by technique reasons.Methods Liver scanning was performed with a high frequency transducer to measure the velocity acceleration time and resistant index of portal vein and hepatic artery after finishing the anastomosis.Results (1)Color flow duplex scanning could detect the hemodynamic aspects of the portal vein and the hepatic artery in intraoperative transplantation;(2)The hepatic artery peak velocity should be >25 cm/s and portal vein mean velocity > (45±16 5)cm/s;(3)The hepatic artery acceleration time should be <0 07s and resisitant index > 56;(4)The difference before and after ligating the shunt veins could be compared.Conclusions Intraoperative color Doppler scanning is the best way to ensure the blood perfusion of the liver graft.
