2型糖尿病踝肱指数和脉搏波传播速度的临床研究

摘　　要：目的　分析影响 2型糖尿病 (T2DM )患者踝肱指数 (ABI)和脉搏波传播速度 (PWV)的因素。　方法　 10 0例T2DM患者分为两组 :T2DM合并高血压 (DH组 ) 72例 ,正常血压 (DNT组 ) 2 8例 ;对照组 5 0例。应用动脉硬化诊断仪测量ABI和PWV。　结果　 (1)ABI的危险因素为性别 (OR =6 1± 0 2 6 ,P =0 0 0 9) ,空腹血糖(OR =7 4± 0 6 3,P =0 0 0 0 7)。 (2 )T2DM组PWV均比对照组快 (P <0 0 1) ,DH组PWV比DNT组快 (P <0 0 1) ,DH组脉压差比DNT组高 (P <0 0 1) ,男女患者PWV无差别 (P >0 0 5 ) ,3组间血脂差异无显著性 (P >0 0 5 )。PWV与年龄正相关 (r =0 4 87,P <0 0 1) ,PWV与收缩压和舒张压呈明显的正相关 (r1=0 70 0 ,r2 =0 4 89;P1<0 0 1,P2 <0 0 1) ,PWV与体质量指数负相关 (r =- 0 2 2 3,P <0 0 5 )。多元逐步回归分析显示 ,收缩压和体质量指数是 2型糖尿病患者PWV的影响因素。　结论　空腹血糖高的女性患者ABI值低 ;高龄、高血压和消瘦为PWV的危险因素。Objective To analyze the influential factors of ankle brachial index(ABI) and pulse wave velocity(PWV) in type 2 diabetic mellitus(DM) patients. Methods One hundred type 2 DM patients were divided into 2 groups: DM with hypertension group(DH, n=72), DM with normotension group(DNT, n=28), fifty healthy as normal control group(CON, n=50). ABI and PWV were measured with Japanese VS-1000 arteriosclerosis diagnometer. Results PWV in type 2 diabetic groups was higher than that in CON groups, and DH groups was higher than that in DNT groups(all P<0.01). Pulse pressure in DH groups was higher than that in DNT groups. There were no significant gender differences in PWV(P>0.05). No significant differences of lipid content were found(P>0.05). PWV was positively correlated with age, SBP and DBP, whereas PWV was negatively correlated with BMI(r=0.487,r=0.700,r=0.489 and r={-0.223}, respectively). In multiple stepwise regression analysis with PWV as an objective variable, SBP and BMI being two effective factors. Conclusion High fast glucose level is the risk of lower ABI in females. The risk factors of PWV were older age, higher SBP and lower BMI.
