摘  要:目的研究乌司他丁(ulinastatin)对颅脑手术患者炎性反应的影响。方法择期颅脑手术患者40例随机分为对照组(C组,n=20)和乌司他丁组(U组,n=20)。U组在麻醉诱导后手术开始前30 min以乌司他丁3 000 IU/kg加入生理盐水100 ml中静脉滴注。分别于手术前、切开硬脑膜后30 min和手术毕抽取动脉血,测定血清白细胞介素-6(IL-6)和肿瘤坏死因子α(TNF-α)水平。结果 术毕两组患者血清IL-6和TNF-α明显较术前高(P<0.01),与C组比较,U组切开硬脑膜后及术毕血清IL-6明显降低(P<0.01),TNF-α亦降低(P<0.05)。结论乌司他丁在减轻全身炎症反应的同时,也能抑制颅脑手术后促炎性细胞因子的过度诱导,从而在颅脑手术患者具有脑保护作用。Objective To investigate the effects of ulinastatin on inflammatory response in patients undergoing cerebral operation. Methods Fourty ASA Ⅰ-Ⅱ patients undergoing cerebral surgery were randomly divided into control group (group C,n = 20)and utinastatin group (group U,n=20). Ulinastatin 3 000 IU/kg was infused in 100 ml of normal saline before operation. Arterial blood samples were taken for determination of IL-6 and tumor necrosis factor-α(TNF-α) before operation,at 30 min after opening the dura mater encephali, and at the end of operation. Results Compared with the baseline, IL-6 and TNF-α were all significantly increased in both groups. In group U, at 30 min after cutting the dura mater encephali and the end of operation, IL-6 was significantly lower than that in group C,so was the TNF-α. Conclusion Utilinastatin may suppress the overproduction of inflammatory cytokines and have a cerebral protective effect in patients undergoing neurosurgery.
