摘 要:背景:一氧化氮在烧伤创面愈合过程起重要作用,针灸有消炎和免疫调节作用,但其作用机制是否对创面组织一氧化氮的影响有关还不清楚。目的:探讨针刺减轻烧伤创面早期进行性损害的机制。设计:双盲随机对照研究。地点和对象:实验在广州市红十字会医院烧伤整形科完成,对象为22只新西兰大白兔,体质量2.0~2.5kg,雌雄不限,由广东省实验动物中心提供。干预:建立兔深Ⅱ度烧伤模型,针刺双侧足三里穴。采用一氧化碳试剂盒(酶法)测定组织一氧化碳。主要观察指标:创面组织一氧化氮的含量。结果:兔深Ⅱ度烧伤早期组织中一氧化氮含量显著低于正常组织。正常组织,烫伤后1,3,5d创面组织一氧化氮分别为(4.76±1.31),(2.68±0.92),(2.07±0.81),(2.85±1.12)μmol/g。以伤后第3天最低(t=2.382,P<0.05);针刺治疗3d,创面组织一氧化氮为(3.05±1.16)μmol/g,明显提高(t=2.216,P<0.05)。但仍低于正常组(4.76±1.31)μmol/g(t=2.156,P<0.05)。结论:针刺能提高创面组织一氧化氮含量,可能是其减轻组织水肿和烧伤创面早期进行性损害重要因素。BACKGROUND:Nitric oxide(NO) plays an important role in the process of wound healing.Acupuncture has anti inflammatory and immunoregulative actions.However,it remains unclear whether its mechanism is related to its effect on NO content in the wound tissue. OBJECTIVE:To study the effect of acupuncture in lessening progressive damage of wound tissue in early burn. DESIGN:A double blind randomized controlled trial was conducted. SETTING and PARTICIPANTS:The study was conducted in the Department of Burn and Plastic Surgery,Guangzhou Red Cross Hospital.A total of 22 New Zealand white rabbits(provided by Center for Experimental Animals of Guangdong Province) of either gender weighing 2.0 kg to 2.5 kg were used for this experiment. INTERVENTIONS:The rabbit model of deep second degree burn was established.Acupuncture was performed on Zusanli point.The content of NO in wound tissue was assayed by enzyme linked immunosorbent. MAIN OUTCOME MEASURES:The content of NO in wound tissue. RESULTS:In the early period of deep second degree burn,the content of NO in wound tissue was lower than that in normal tissue.The content of NO was (4.76±1.31),(2.68±0.92),(2.07±0.81) and (2.85±1.12) μmol/g,respectively,in normal and wound(at days 1,3 and 5 after burn) tissues,and it was the lowest at day 3 after burn(t=2.382,P< 0.05).The content of NO[(3.05±1.16) μmol/g]was significantly increased in wound tissue after 3 day acupuncture(t=2.216,P< 0.05),but was still lower than that in normal tissue(t=2.156,P< 0.05). CONCLUSION:Acupuncture can increase the content of NO in wound tissue and is probably an important factor for alleviating tissue edema and progressive damage of wound tissue.
