30例房间隔缺损封堵器治疗效果评价

摘　　要：目的　应用房间隔封堵器治疗继发孔型房间隔缺损 (ASD) ,并对其疗效进行初步评价。方法　全组共 30例 ,年龄3～ 5 3岁 ,平均 (2 7± 4 )岁。在X线透视及超声心动图监视下从右股静脉经导管置入封堵器封堵房间隔缺损。术后即刻、2 4h、1个月、3个月、6个月分别行经胸超声心动图评价治疗效果。结果　全组疗效均佳 ,手术 3个月后未见残余分流 ,成功率 10 0 %。ASD封堵治疗术后 6个月右室舒张末期前后径、右室舒张末期容积、右室收缩末期容积均较术前明显缩小 (P <0 0 5 )。结论　应用房间隔封堵器是治疗继发孔型房间隔缺损的有效方法 ,操作简便 ,成功率高 。Objective To evaluate the short-term and long-term therapeutic effectiveness on transcatheter closure of ostium secundum atrial septal defect (ASD) using atrial septal occluder. Methods Thirty patients, aged 3～53 years with a mean of (27±4) years, underwent the transcatheter closure operation for ostium secundum atrial septal defect (ASD) using atrial septal occluder under the guidance of fluoroscopy and transthoracic echocardiography. Transthoracic echocardiography was performed immediately after operation and was done 24h、 1 month 、3 months and 6 months following deployment of the umbrella-like occluder device from right femoral venous approach to evaluate the curative effectiveness. Results The treatment efficiency of 30 patients was satisfied. No residual shunt was found in all patients 3 months after the procedures. The successful rate 100%. After transcatheter closure, right ventricular end diastolic diameter(RVEDD), right ventricular end diastolic volume(RVEDV), right ventricular end systolic volume(RVESV) decreased significantly 6 months after operation. Conclusion ASD patients with small shunts may live a normal span and do not require surgery. Larger shunts may cause disability by age 40, surgery closures are indicated. Transcatheter closure of ostium secundum ASD using occluder is an efficient technique to cure the disorders, possesses high successful rate and showes immediate and long-term outcome.
