摘　　要：目的　观察喉全切除术后应用Blom -Singer发音管重建喉发声功能的效果。方法　回顾性分析 1994年 9月～ 2 0 0 3年 8月 15例喉全切除术后行Blom -Singer发音管重建喉发声功能患者的临床资料。结果　 15例患者中 12例行Ⅰ期发音管重建术 ,10例手术成功 (10 / 12 ,83.33% ) ;3例行Ⅱ期发音管重建术 ,其中 2例手术成功 ,两种术式的总成功率为 80 % (12 / 15 )。Ⅰ期手术失败 2例 ,与气管食管壁分离过多有关 ;Ⅱ期手术失败 1例 ,系环咽肌切断不完全所致。结论　喉全切除术后应用Blom -Singer发音管重建喉发声功能是一种有效的发声重建方法 。Objective To study the effect of Blom-Singer's prosthesis for voice rehabilitation following total laryngectomy.Methods 15 patients with rehabilitated speech by Blom-Singer's prosthesis after total laryngectomy from 1994 to 2003 were analyzed retrospectively.Results Voice restoration was achieved in ten of twelve patients who installed Blom-Singer's prosthesis at primary operation and two of the other three patients at secondary operation.The success rate of rehabilitation of the voice was 80 percent.Dissociating tracheoesophageal wall too much and incising cricopharyngeal muscle incompletely resulted in surgery failure.Conclusion In those patients with total laryngectomy,Blom-Singer's prosthesis speech appears to be an effective form of rehabilitation.Cricopharyngeal myotomy and keeping tracheoesophageal wall intacted are the key point for voice rehabilitation by Blom-Singer's prosthesis.
