摘  要:目的:探讨肝组织胰岛素样生长因子-1(insulinlikegrowthfactor-1,IGF-1)基因表达异常及其与糖尿病外周神经病变的关系。方法:实验选用清洁级SD雄性大鼠64只。将64只大鼠按初质量匹配随机分成:正常对照组16只和糖尿病组48只。用四氧嘧啶诱发糖尿病大鼠模型,并进一步将糖尿病大鼠按血糖水平分成3组(胰岛素治疗1,2,3组),与正常大鼠组进行实验比较。反转录一多聚酶链扩增反应半定量分析坐骨神经IGF-1mRNA含量;酶联免疫吸附法分析组织IGF-1肽含量;诱发肌电图测定坐骨神经电生理指标;光镜,电镜下观察坐骨神经形态学改变。结果:糖尿病2周时,胰岛素治疗2,3组大鼠肝组织IGF-1mRNA含量明显低于正常对照组,且随病程发展进一步下降(P<0.01)。糖尿病3个月时,胰岛素治疗2,3组大鼠肝组织IGF-1mRNA含量与正常对照组比较,差异有非常显著性意义(P<0.01)。肝组织IGF-1肽含量与IGF-1mRNA几呈平行变化趋势。此变化趋势与肝组织IGF-1肽含量变化一致(r=0.99,P<0.001)。糖尿病2周时,糖尿病组与正常对照组间电生理差异无显著无意义(P>0.05)。糖尿病2,3个月时,胰岛素治疗2,3组大鼠电生理指标与正常对照组比较,差异有显著性意义(P<0.05~0.01)。糖尿病2,3个月时,胰岛素治疗3组与胰岛素治疗2组比较,差异有显著性意义(P<0.01),胰岛素?AIM:To explore the liver tissue insulin like growth factor 1(IGF-1) gene expression abnormality, and its relationship with diabetic peripheral neuropathy. METHODS:Sixty-eight male cleaning SD rats were randomly divided into normal control group(n=16) and diabetes mellitus(DM) groups(n=48) according to initially matched body mass. DM rat models induced by alloxan were subdivided into insulin treatment groups 1,2 and 3 according to their blood glucose levels,which were compared with normal control group.IGF-1 mRNA content in sciatic nerve was quantitatively analyzed by reverse transcriptase-polymerase chain reaction(RT-PCR),IGF-1 peptide content in tissue was analyzed by enzyme-linked immunosorbent assay(ELISA),electrophysiological parameters of sciatic nerves were determined by evoked electromyogram,and morphometric changes of sciatic nerves were observed under light microscope and transmission electron microscope. RESULTS:Two weeks after diabetes, the IGF-1 mRNA contents in liver tissue of rats in the insulin treatment groups 2 and 3 were obviously lower than that in the normal control group, and were reduced further with the development of the disease(P< 0.01).Three months after diabetes, the IGF-1 mRNA contents in liver tissue of rats in the insulin treatment groups 2 and 3 had very significant difference as compared with that in the normal control group(P< 0.01),and IGF-1 peptide contents in liver tissue had paralleled changing tendency with the IGF-1 mRNA ones,which was coincident with the changes of IGF-1 peptide in liver tissue(r=0.99,P< 0.001).There were insignificant differences in the electrophysiological indexes between the normal control group and the diabetic group 2 weeks after diabetes(P >0.05),while there were significant differences 2 and 3 months after diabetes(P< 0.05 to 0.01),and there was also significant difference between the insulin treatment groups 2 and 3(P< 0.01),while insignificant difference was observed between the insulin treatment group 1 and normal control group(P >0.05).
