摘  要:为了评价新生儿出生后48h内经皮胆红素(TCB)水平对新生儿病理性黄疸的预测价值,本文测定了359例正常足月混合喂养新生儿出生后0~48h,3~4d和5~6d的TCB读数,结果:(1)随出生时(出生6h内)TCB读数的增加,病理性黄疸发生率逐渐升高,当TCB读数≤6和≥9时,病理性黄疸发生率分别是11.32%和65.22%。(2)出生时TCB读数均值为7.11±1.24.而后每小时以0.2的速度递增,24h后递增速度减慢,但病理性黄疸发生率无明显改变。(3)出生后0~6,7~12,13~18,19~24和25~48hTCB读数对新生儿病理性黄疸均有预测作用,分别取TCB读数为9,10,11,12和15时预测价值好,提示:出生后48h内任意时间测定TCB读数对新生儿病理性黄疸均有预测作用,且以接近24h时预测价值最佳。Transcutaneous bilirubin(TCB) measurement was performed on 359 cases normal full-term mixed-fed newborn infants during the first 0~48 hours,3~4days and 5~6 days of life. The results are(1) the rate of pathologic jaundice of newborn increases continuously with the increases of TCB readings(during the first 6 hours of life). When TCB readings was less than or equal to 6 and more than or equal 9, the rate of pathologic jaundice were 11.32% and 65.22% respectively. (2)The mean of TCB readings was 7. 11±1.24 at birth, followed by an increase of 0.2 per hour. and the increase became slower after the first 24 hours of life, but the rate of pathologic jaundice was not evidently changed. (3) Any TCB readings during the first 0~6,7~12, 13~18, 19~24 and 25~48 hours of life may predict the pathologic jaundice of newborn infants, and the prediction was more accurate when TCB reading was 9,10,11,12 or 15. Our study revealed that any TCB reading during the first 48 hours of life can predict the pathologic jaundice, and that the prediction was accuratest at about 24 hours of life.
