摘  要:目的探讨多层螺旋CT多平面重建技术(multiplanreconstruction,MPR)对听骨链重建术后效果评估的应用价值。方法回顾分析6例慢性化脓性中耳炎及外伤性听骨链中断患耳,分别采用自体听小骨、羟基磷灰石和钛质听骨赝复物作为听骨链重建材料,术后用多层螺旋CT薄层扫描(层厚1mm)并行多平面重建(重建层厚1mm)检查听骨链。结果术后听小骨及听骨链显示清晰,人工听骨及自体听小骨与周围毗邻结构的连接关系显露良好,尤其是使用自体改形听小骨和羟基磷灰石听骨赝复物重建之听骨链显示较为清晰。结论采用多层螺旋CT薄层扫描并行多平面重建,能从任意角度清晰显露听骨链的形态和位置,为评估听骨链重建术后状况提供直接参考依据。Objective To investigate the value of multislice spiral CT with multiplan reconstruction (MPR) in the assessment of the ossicular chain reconstruction. Methods The authors reviewed and analysed the materials of four cases of chronic suppurative otitis media and 2 cases of traumatic interruption of the ossicular chain, treated with modified radical tympanomastoidectomy and simple tympanoplasty, both with ossicular chain reconstruction, using autogenous ossicles, hydroxyapatite, and titanium prostheses. The spiral CT scanning with thin slice (1 mm) was performed after the operation, followed by multiplan reconstruction (MPR). Results The shape and position of the reconstructed ossicular chain as well as its joint to the neighbor structure were clearly imaged using MPR, especially in the case of that using hydroxyapatite prostheses and autogenous ossicles, probablely because of the similar density of these materials to the neighbor bone structure. Conclusion Because of the high resolution and the free direction of the reconstruction, spiral CT with MPR can get clear image of the reconstructed ossiclar chain, including the position and shape, which can provide a direct evidence for the assessment of the operation.
