中国部分地区大肠埃希菌和肺炎克雷伯菌超广谱β内酰胺酶基因型研究

摘　　要：目的　了解中国产超广谱 β内酰胺酶 (ESBLs)大肠埃希菌和肺炎克雷伯菌的ESBLs基因型分布。方法　收集北京、浙江、新疆及郑州等共 4 0 0株临床分离的产ESBLs大肠埃希菌和肺炎克雷伯菌 ,采用接合试验 ,聚合酶链反应 ,克隆测序等方法明确ESBLs基因型。结果　 4 0 0株细菌中 35 6株细菌ESBLs的基因型明确。其中 32 8株细菌产 1种ESBLs,主要为CTX M型 ,包括CTX M 14型ESBLs占 5 2 2 5 % ,CTX M 3型占 14 2 5 % ,CTX M 2 4型占 7 2 5 % ,CTX M 2 2型占 2 % ,CTX M 2 8和CTX M 9型分别占 0 75 % ,CTX M 13、CTX M 2 7和CTX M 2 9型各占 0 2 5 % ;其次为SHV型 ,其中SHV 12占2 5 % (10 / 4 0 0 ) ,SHV 5占 1 0 0 % ,SHV 2占 0 5 %。 2 8株细菌同时携带 2种或 3种ESBLs ,占 7 0 0 %(2 8/ 4 0 0 ) ,其中以同时产CTX M 14和CTX M 3最常见。结论　中国产ESBLs大肠埃希菌和肺炎克雷伯菌的ESBLs基因型以CTX M 14型为主 ;其次为CTX M 3型 ,部分菌株同时产两种及两种以上ESBLs。Objective To investigate the genotype distribution of extended-spectrum β-lactamases (ESBLs) produced by Escherichia coli and Klebsiella pneumoniae in ChinaMethods 400 clinical strains of Escherichia coli and Klebsiella pneumoniae confirmed to produce ESBLs were collected from four areas of China Plasmid conjugation, PCR, gene cloning and DNA sequencing were carried outResults 356 of 400 isolates confimed the genotype of ESBLs 328 strains produced only one type of ESBLs, including CTX-M-14 (209 strains), CTX-M-3(57 strains), CTX-M-24(29 strains), CTX-M-22 (8 strains), CTX-M-28 (3strains), CTX-M-9(3 strains), CTX-M-13 (1 strain), CTX-M-27 (1 strain), CTX-M-29 (1 strain); SHV-12(10 strains), SHV-5(4 strains), SHV-2 (2 strains)。Twenty-eight isolates carried two or three types of ESBLs, producing CTX-M-14 and CTX-M-3 together which were the most common typeConclusion CTX-M-type is the most common genotype in ESBLs produced by Escherichia coli and Klebsiella pneumoniae in ChinaCTX-M-14 was the predominant genotype Some isolates produced two or three ESBLs
