支气管哮喘患儿体液免疫功能及血浆白细胞介素-6和γ-干扰素的变化

摘　　要：目的 :探讨支气管哮喘患儿体液免疫、补体变化及血浆白细胞介素 - 6和 γ-干扰素与哮喘的病情程度之间的关系。方法 :对 37例支气管哮喘患儿进行外周血体液免疫及补体含量检测 ,同时测定急性发作期治疗前后血浆 IL- 6与 IFN- γ水平 ,与 34例健康对照组比较。结果 :支气管哮喘患儿外周血 Ig G、C3、C4明显降低 ,Ig E明显升高 ,Ig M、Ig A无明显变化。哮喘急性发作期血浆 IL- 6水平高于对照组 ,临床缓解期仍高于对照组。哮喘急性发作期血浆 IFN- γ水平低于对照组和临床缓解期 ,差异具有显著性 (P<0 .0 1 )。结论 :支气管哮喘存在着体液免疫功能紊乱 ,在哮喘发作期和缓解期均有血浆 IL- 6与 IFN- γ平衡失调 ,Th1 / Th2失衡在支气管哮喘发病机理中占重要地位。Objective:To study the changes of the humoral immunity and to investigate the changes of plsma interleukin-6 (IL-6) and interferon-γ(IFN-γ) levels in children with bronchial asthma, and the relationship between the cytokines levels and the serious extent of the disease.Methods:Tests of the blood humoral immunity and complement content in 37 bronchial asthma cases were carried out . Their plasma IL-6 and IFN-γ levels were determined respectively in actue exacerbating asthma as actue exacerbating group (AEG) and after remission as clinical remitting group (CRG), then were compared with 34 healthy controls.Results:In the blood of the children involved, there was an obvious rise of IgE as well as a drop of IgG, C3, C4, and no apparent change of IgA.The plasma IL-6 level in AEG was higher than that in controls.The IL-6 level in CRG was lower than that in AEG, and both were higher than that in controls. The plasma IFN-γ level in AEG was lower than that in controls and in CRG.Conclusion:Children with bronchial asthma have an humoral immunity disturbance. There exists imbalance between IL-6 and IFN-γ in the stage of both attack and convalescence. The ratio imbalance of Th1 and Th2 plays an important role in the development mechanism of bronchial asthma.
