动脉插管介入治疗股骨头缺血性坏死68例  

Therapeutic observation on Interventional Therapy of Arterial Catheterization for 68 Cases with Avascular Necrosis of the Femoral Head

摘　　要：[目的 ]探讨应用导管经旋股内、外动脉、闭孔动脉灌注治疗股骨头缺血坏死的临床疗效。 [方法 ]介入溶通治疗股骨头缺血坏死 6 8例 ,10 8髋 ,对治疗前后疼痛、关节活动度进行比较 ,观察分析临床疗效。 [结果 ]6 8例患者临床症状明显改善 ,髋关节疼痛、功能障碍得到缓解或消失。治疗后X线复查坏死骨质均有不同程度的吸收 ,新骨形成及修复。 [结论 ]介入治疗能改善股骨头的血液循环 ,是股骨头缺血坏死有效的治疗方法 ,Ⅰ、Ⅱ期的疗效优于Ⅲ和Ⅳ期。[Objective]To explore the therapeutic effect of the treatment of ANFH by applying catheter into circumflex internal and external femoral arteries and obturator artery.[Methods]108 hips in 68 patients of ANFH were treated by Intervention. The scores of pain, activity of joints were compared, and the clinical effects were observed and analyzed before and after the treatment.[Results]There was improvement of clinical symptoms with relief or disappearance of pain and functional disorder in hip joint in the 68 patients. Reexamination with X-ray after treatment showed reabsorption of necrotic bone in different degrees,formation and repair of new bone.[Conclusion]Interventional therapy for the circulation of femoral head is an effective treatment,and the effect of the treatment in the Ⅰ、Ⅱ stage is better than that in the Ⅲ and Ⅳ stage.
