脑脊液检测LAM-IgG与TB-IgG实验诊断结脑的比较

摘　　要：目的 :比较检测脑脊液 (CSF)标本抗阿拉伯糖甘露糖脂IgG抗体 (LAM IgG)和抗结核杆菌IgG抗体 (TB IgG)诊断结核性脑膜炎 (结脑 )的价值。 方法 :应用斑点免疫金渗滤试验检测CSF中LAM IgG和TB IgG ,其中结核性脑膜炎 (结脑组 ) 132份、对照组 5 5份。应用卡方检验比较两种方法检测结果的差别。结果 :结脑组检测LAM IgG和TB IgG的阳性结果分别为 6 8份 (5 1.5 % )和 5 3份 (4 0 .2 % ) ,对照组无阳性结果 ,两个指标检测结果的差别具有极显著性 (P <0 .0 1)。结论 :在诊断结脑方面 ,LAM IgG指标的价值高于TB IgG。检测CSF中LAM IgG有助于结核性脑膜炎的诊断。Objective:To compare the value between LAM-IgG and TB-IgG in CSF for the diagnosis of tuberculous meningitis. Methods:LAM-IgG and TB-IgG were detected by dot immunogold filtration assay (DIGFA) in CSF (including 132 cases of tuberculous CSF and 55 cases of control CSF). The difference of detecting results between LAM-IgG and TB-IgG was compared by Chi-square test. Results:The positive results of LAM-IgG and TB-IgG were 68(51.5%) and 53(40.2%) in tuberculous CSF. In the control CSF, the results of LAM-IgG and TB-IgG were all negative. The difference of the results between LAM-IgG and TB-IgG had statistical meaning (P<0.01). Conclusion:LAM-IgG is a useful index and its value is higher than TB-IgG in the diagnosis of tuberculous meningitis.
