摘 要:目的探讨通过聚合酶链反应(PCR)方法检测上呼吸道病变组织中的EB病毒(EBV)感染情况在韦格纳肉芽肿病(WG)和鼻T/NK细胞淋巴瘤鉴别诊断中的价值。方法提取11例WG、16例鼻T/NK细胞淋巴瘤和15例慢性鼻窦炎或鼻腔息肉患者的上呼吸道病变活检组织石蜡包埋标本的DNA,应用PCR方法检测其中是否存在EBV-DNA。结果各组的EBV-DNA阳性率分别为:WG组10.0%,鼻T/NK细胞淋巴瘤组60.0%,慢性鼻窦炎或鼻腔息肉组9.1%。WG组和鼻T/NK细胞淋巴瘤组的阳性率比较差异具有统计学意义(P=0.018)。结论WG与EBV感染无关,检测上呼吸道病变组织的EBV-DNA有助于鉴别诊断WG和鼻T/NK细胞淋巴瘤。Objective To evaluate the clinical significance of EB virus (EBV) detectation in upper respiratory tract tissue by polymerase chain reaction (PCR) in differentiating Wegener′s granulomatosis (WG) and nasal T/NK-cell lymphoma. Methods DNA was extracted from paraffin embedded biopsy spec-imens of upper respiratory tracts of 11 WG, 16 nasal T/NK-cell lymphoma and 15 chronic naso-sinusitis or nasal polyps patients. EBV-DNA was detected by PCR. Results The positive rate of EBV-DNA was 10.0% in WG group, 60.0% in nasal T/NK-cell lymphoma group and 9.1% in chronic naso-sinusitis or nasal polyps group respectively. The difference between WG and nasal T/NK-cell lymphoma group had statistical significance (P=0.018). Conclusion WG is not associated with EBV infection. Detecting EBV-DNA in upper respiratory tract tissue lesion is helpful for differentiating WG and nasal T/NK-cell lymphoma.
