多发性肌炎/皮肌炎患者血清TNF-α及sFas变化的临床意义

摘　　要：目的　通过测定多发性肌炎 (PM) /皮肌炎 (DM)患者血清肿瘤坏死因子 (TNF- α)及血溶性 Fas(s Fas)水平 ,探讨二者在 PM/DM发病过程中的免疫病理作用。方法　采用酶联免疫吸附双抗体夹心法(EL ISA)检测 2 2例 PM/DM患者及 18例健康人血清 TNF-α、s Fas,及血清磷酸激酶 (CPK)水平。结果　 PM/DM患者血清 s Fas水平明显高于正常人 (P<0 .0 5 ) ,且与血清 CPK成正相关 (P<0 .0 1)。 PM、DM间血清 s Fas水平无明显差别 (P>0 .0 5 )。 2 2例 PM/DM患者中 ,8例测到 TNF- α,正常人未测到 TNF- α。PM/DM测出 TNF- α者与未测出 TNF- α者 CPK水平无明显差别 (P>0 .0 5 )。结论　s Fas可能参与了 PM/DM的免疫病理过程 ,并随病情加重而增高 ,TNF- α同样参与了 PM/DM的免疫病理过程 ,但是否同时具有免疫上调和免疫下调两种作用 。Objective To detecte the serum levels of tumor necrosis factor-α(TNF-α)and soluble Fas (sFas) from 22 patients with PM/DM and 18 normal controls,probe the values of them in the immunopathological process of PM/DM.Methods The serum levels of tumor necrosis factor-α (TNF-α) and soluble Fas (sFas) from 22 patients with PM/DM and 18 normal controls were detected with enzyme linked immunosorbent assay.At the same time,the serum level of CPK was detected.Results The serum level of sFas was higher than that of the normal controls (P<0.05),and it has a positive correlation with the serum level of CPK(r=0.59,P<0.01). There is no difference in serum levels of sFas between polymyositis and dermatomyositis (P>0.05).Conclusion sFas may play an important role in the immunopathological process of PM/DM.Whether it can both improve and inhibit immune reaction at the same time has to be further studied yet.
