妊高征患者外周血Th_1/Th_2失衡与细胞因子IL-12的相关性研究

摘　　要：目的 :探讨妊娠高血压综合征 ( pregnancy induced hypertension,PIH)患者外周血 Th1 / Th2 失衡与 IL - 12的关系及 Th1 / Th2 比率失衡的原因。方法 :采集 18例重度 PIH患者、 13例轻中度 PIH患者、 15例正常妊娠妇女及 15例健康未孕妇女的外周血淋巴细胞 ,用酶联免疫吸附法 ( EL SIA)测定 IL - 12水平 ,用流式技术测 Th1 / Th2 比率。结果 :重度 PIH患者外周血 IL- 12 ( 89.83± 6 .2 4 pg/ ml)水平显著高于其他各组 ( P<0 .0 0 1) ,Th1 / Th2 比率 ( 18.2 6± 1.2 4 )亦显著高于其他各组 ( P<0 .0 0 1) ,且二者存在明显正相关 ( r=0 .6 72 ,P<0 .0 0 1)。结论 :PIH患者其外周血单核淋巴细胞 ( peripheral blood mononuclearcell,PBMC)分泌的 IL - 12可引起 Th2 优势向 Th1 优势转变 ,从而导致 Th1 / Th2Objective:To investigate the relation between interleukin 12 (IL-12) concentration and imbalance in the T—helper1/T—helper2 (Th 1 /Th 2 ) ratio in peripheral blood of patients with pregnancy induced hypertension (PIH) and to discuss the reason of the imbalance. Methods:Peripheral blood mononuclear cell (PBMC) were collected from 18 patients with severe PIH, 13 patients with mild or moderate PIH, 15 healthy pregnant women, 15 healthy non-pregnant women. IL-12 concentration was determined by enzyme-linked immunosorbent assay (ELISA), Th 1 /Th 2 ratio in PBMC were determined by flow cytometry.Results:PBMC from severe PIH patients secreted more IL-12 (89.83±6.24 pg/ml) than that from other groups ( P <0.001), the ratio of Th1/Th2 (18.26±1.24) was much higher than that from other groups ( P <0.001), and Th 1 /Th 2 ratio were correlated positively to IL-12 ( r =0.672, P <0.001). Conclusion:Increased IL-12 secretion by monocytes from PBMC may cause shift from Th 2 dominance to Th 1 dominance in patients with PIH and lead to imbalance of Th1/Th2 ratio.
