腹水超滤·浓缩回输系统临床应用评价

摘　　要：目的探讨腹水超滤浓缩回输腹腔系统的临床应用价值。方法对12例肝硬化顽固性腹水患者在左下腹、右中腹进行腹腔穿刺,并将穿刺针与导管连接固定,腹水从左下腹引入动脉导管,设定正压泵流速为150～250ml/min,通过滤过器在负压泵作用下形成超滤,水及中、小分子等滤出,浓缩后的腹水经静脉导管自右上腹回输入腹腔。结果超滤后患者体重下降,腹围缩小,尿量增加,血中肌酐下降,总蛋白、白蛋白上升(P<0.05),腹水中总蛋白增加(P<0.05),补体C3浓度下降(P<0.05),钾、钠、氯无显著差异(P>0.05)。结论腹水超滤回输有效、安全、简便、副作用少。Objective:To determine the clinical application value of the Ascites Ultrafiltration Concentration.Methods:Do abdominocentesis for 12 cases hepatocirrhosis with stubbom ascites,back-transfussion the ascites ultrafiltrated and concentrated into their periloneal cavity.Results:After the cure,the patients' weight is lighten,abdominal circumference is shorten,urine is increased,serum creatinine is decreased,serum total proteins and albumin is increased (P < 0.05),ascites total protein is increased(P < 0.05)and alexin C too (P < 0.01),while potassium,sodium and chlorin almost no change (P >.05).Conclusion:The ascites ultrafiltration concentration is effect,safe,convenient and with little side effect.
