测定T细胞亚群、细胞因子、C反应蛋白对判断移植受者免疫状态的价值

摘　　要：目的 探讨综合外周血T细胞亚群(CD_4^+、CD_8^+)、细胞因子(IL-4、IFN-γ)、C-反应蛋白(CRP)测定对判断移植受者不同免疫状态(抑制、耐受、排异)的价值。方法 采用流式细胞计数测定CD_4^+、CD_8^+细胞百分比,采用双抗体夹心ELISA法测定IL-4、IFN-γ水平,采用散射比浊法测定CRP水平。结果 免疫抑制组CD_4^+、CD_8^+细胞百分比及比值、CRP均与对照组无显著差异,而IL-4、IFN-γ均显著下降,其比值反而升高;免疫排斥组CD_4^+、CD_8^+细胞百分比、IL-4、IFN-γ、CRP均较对照组升高,而IL-4、IFN-γ比值却下降;免疫耐受组CD_8^+细胞百分比升高,因此CD_4^+/CD_8^+降低,IL-4升高而IFN-γ下降,CRP无变化。结论 综合T细胞亚群、细胞因子、C反应蛋白测定可作为判断受者免疫状态的指标。Purpose: To study the values of determination of T cell subsets(CD4+,CD8+, cytokines(interleukin-4, interferon-γ) and c-reactive protein in indication of different immunostate (suppressive, rejective, tolerant) of the recipients. Methods: FITC was used to determine the T cell subsets CD4+, CD8+ cells in peripheral blood. Double-antibody-coated ELISA was used to measure the level of IL-4 and IFN-γ in supernatants from MLC which using the lymphocytes from the donors and the recipient's. Scatter nephelometry was used to determine the level of CRP. Results: In immunosuppresive group, the percentages of CD4+ and CD8+ cells, CD4+/CD8+ and the level of CRP had no difference from the control group; levels of IL-4 and IFN-γ were all reduced but IL-4/IFN-γ rose. In rejective group, percentages of CD4+, CD8+ cells rose, but their ratio was no difined; levels of IL-4, IFN-γ also rose in contrast with the control group, but IL-4/IFN-γ reduces; level of CRP rose significantly. In immunotolerant group, percentage of CD8+ cell rose, so CD4+/CD8+ reduced; level of IL-4 rose and IFN-γ reduced, thus mmade the ratio of them greatly higher; level of CRP had no changes. Conclusions: The precentage of CD4+, CD8+ cells and their ratio, levels of IL-4, IFN-γ and their ratio, and the level of CRP together can be used to determine the immunostate of the recipients.
