右心房双心室起搏治疗充血性心力衰竭的临床应用(附2例报告)

摘　　要：目的　观察右心房双心室起搏治疗扩张型心肌病并心力衰竭的临床效果 ,探讨其治疗机制。方法　两例患者均为男性 ,诊断为扩张型心肌病 ,心功能Ⅲ和Ⅳ级 (NYHA)。例 1并完全性左束支传导阻滞。两例患者均行右心房双心室起搏 ,例 1用InsyncTM 80 4 0三腔起搏器 ,左心室电极为 2 187～ 75cm电极 ,通过冠状静脉窦植入冠状静脉左室支。例 2用SIGMATMSD2 0 3起搏器 ,左心室起搏通过冠状静脉窦植入 2 188电极导线 ,于冠状静脉左室支远端进行起搏 ,左右心室电极导线通过Y型转换器与起搏器连接。术前、术后 1周和 6个月进行血流动力学及心功能评价。结果　右心房双心室起搏后两例患者血流动力学指标及心功能均改善。Objective: To observe the effect of treating congestive heart failure(CHF) in patients with dilated cardiomyopathy(DCM) by right atrial and biventricular pacing Methods: Two DCM cases (both males at an average age of 54), with NYHA class Ⅲ to Ⅳ CHF(1 case with complete left bundle branch block), received right atrial and biventricular pacing Left ventricular pacing was performed by 2187-75cm lead through the coronary sinus in the case with InsyncTM 8040 tri chamber pacemaker Left ventricular pacing was perfomed by 2188 lead through the coronary sinus in another case with SIGMATMSD203 pacemaker, and left and right ventricular leads were connected with pacemaker by Y type switch The hemodynamic effects and cardiac functions were assessed up to 6 months after the pacing Results: The hemodynamic effects and cardiac functions of the 2 cases were improved with the therapy after the pacing Conclusion: Right atrial and biventricular pacing improve cardiac function in DCM cases with CHF
